益气活血通络汤联合锁定加压钢板内固定对四肢骨折患者恢复情况、血液微循环及生活质量的影响

摘　　要：目的探讨益气活血通络汤联合锁定加压钢板内固定对四肢骨折患者恢复情况、血液微循环及生活质量的影响。方法前瞻性选取2020年1月至2023年7月秦皇岛市第三医院收治的四肢骨折患者200例纳入本研究。按照随机数字表法将其分为观察组和对照组,每组各100例。对照组予锁定加压钢板内固定法,观察组在对照组基础上联合益气活血通络汤干预。两组均持续干预2个月。比较两组肿胀消失时间、骨折愈合时间、创口愈合时间、住院时间,以及对血液微循环状态、Fugl-Meyer评分、日常生活活动能力(ADL)评分、生活质量综合评定问卷-74(GQOL-74)评分的影响。结果观察组肿胀消失时间、创口愈合时间、骨折愈合时间及住院时间分别为(4.29±1.35)、(11.24±3.12)、(5.24±2.74)、(15.37±4.17)、(13.25±2.39)d,均短于对照组[(6.35±2.24)、(14.64±3.64)、(7.59±3.23)、(19.23±5.24)、(16.35±4.15)d],差异均有统计学意义(P<0.05)。治疗2个月后,两组各项血液黏度、形态积分、流态积分均较治疗前降低,襻周积分均较治疗前升高,且观察组的各项血液黏度、甲襞微循环积分指标均低于对照组,差异均有统计学意义(P<0.05)。治疗2个月后,两组Fugl-Meyer评分、ADL评分均较治疗前升高,且观察组的Fugl-Meyer评分、ADL评分分别为(86.64±8.45)、(77.34±9.64)分,均高于对照组[(77.64±7.46)、(67.74±9.83)分],差异均有统计学意义(P<0.05)。治疗2个月后,两组GQOL-74各维度评分均较治疗前升高,且观察组的GQOL-74各维度评分均高于对照组,差异均有统计学意义(P<0.05)。结论益气活血通络汤联合锁定加压钢板内固定可缩短四肢骨折患者的康复时间,降低各项血液黏度和甲襞微循环积分指标,改善血液微循环状态,提高患肢功能,改善患者的生活质量,可在临床推广应用。Objective To investigate the effect of Yiqi Huoxue Tongluo decoction combined with locking compression plate internal fixation on recovery,blood microcirculation and quality of life in patients with limb fractures.Methods In this prospective study,200 patients with limb fractures admitted to The Third Hospital of Qinhuangdao from January 2020 to July 2023 were included in the study.They were divided into the observation group and the control group according to the random number table method,with 100 cases in each group.The control group was given locking compression plate internal fixation method,and the observation group was given Yiqi Huoxue Tongluo decoction on the basis of the control group.Both groups continued the intervention for 2 months.The effects of swelling disappearance,fracture healing,wound healing,hospitalization time,blood microcirculation status,Fugl-Meyer score,activities of daily living(ADL)score,and generic quality of life inventory-74(GQOL-74)score were compared between the two groups.Results The swelling disappearance time,wound healing time,fracture healing time and hospitalization time in the observation group were(4.29±1.35),(11.24±3.12),(5.24±2.74),(15.37±4.17)and(13.25±2.39)d,respectively,which were shorter than those in the control group[(6.35±2.24),(14.64±3.64),(7.59±3.23),(19.23±5.24)and(16.35±4.15)d],and the differences were statistically significant(P<0.05).After 2 months of treatment,the blood viscosity,morphological score and flow state score of the two groups were lower than those before treatment,and the periloop score was higher than that before treatment,the blood viscosity and nailfold microcirculation score of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After 2 months of treatment,the Fugl-Meyer score and ADL score of the two groups were higher than those before treatment,and the Fugl-Meyer score and ADL score of the observation group were(86.64±8.45)and(77.34±9.64)points,respective
