摘  要:目的探究足月小样儿母体胎盘菌群构成及胎儿脐血胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白3(IGFBP-3)水平特征。方法前瞻性选取2021年12月至2023年12月在邢台市中心医院新生儿科出生、住院且儿童保健门诊定期规律随访和管理至满3周岁的128例婴幼儿为研究对象,根据新生儿的出生胎龄和体重的关系,将其分为健康组(n=36)和小样儿组(n=92)。分析母婴临床资料(母亲年龄、孕前体重指数、出生身长、民族、产史、新生儿性别、出生方式、脐动脉pH、脐动脉乳酸、脐静脉pH、脐静脉乳酸)差异及母体胎盘菌群差异,记录其微生物多样性指数(Shannon指数、Simpson指数、Richness指数、Chao指数、ACE指数),并分析其胎盘微生物群落结构情况,并对胎儿的脐血IGF-1、IGFBP-3的组间差异进行分析。结果健康组和小样儿组的母亲年龄、孕前体重指数、民族、产史、新生儿性别、出生方式、脐动脉pH、脐动脉乳酸、脐静脉pH、脐静脉乳酸比较,差异均无统计学意义(P>0.05),小样儿组的出生身长短于健康组,差异有统计学意义(P<0.05)。健康组和小样儿组的Shannon指数、Simpson指数、Richness指数、Chao指数、ACE指数比较,差异均无统计学意义(P>0.05)。健康组中,胎盘微生物以变形菌门、放线菌门、拟杆菌门、厚壁菌门为主,小样儿组以变形菌门、放线菌门、拟杆菌门为主。与健康组相比较,小样儿组的变形菌门、拟杆菌门、厚壁菌门比例均降低,但未分类的细菌比例升高,差异均有统计学意义(P<0.05)。健康组和小样儿组胎盘中以克雷伯杆菌属、多形杆状菌属、代尔夫特菌属、埃希-志贺菌属、Faecalibacterum、草螺菌属、不动杆菌属、乳杆菌属、根瘤菌属占比均较高,小样儿组的普雷沃菌属、丁酸菌属、梭状芽孢杆菌属Ⅺ、小杆菌属、黄曲霉属、链球菌属占比均更高,差异均有统计学意义(Objective To explore the composition of placental microbiota and analyze the characteristics of insulin-like growth factor-1(IGF-1)and insulin-like growth factor binding protein-3(IGFBP-3)levels in the umbilical cord blood of full-term small-for-gestational-age infants.Methods A total of 128 infants and young children who were born and hospitalized in the Department of Neonatology,Xingtai Central Hospital from December 2021 to December 2023 and regularly followed up and managed by the children's health clinic to the age of 3 years were prospectively selected as the research objects.According to the relationship between the birth gestational age and weight of the newborn,they were divided into the healthy group(n=36)and the small sample group(n=92).Differences in maternal and neonatal clinical data(maternal age,pre-pregnancy body mass index,birth length,nationality,birth history,neonatal sex,birth mode,umbilical artery pH,umbilical artery lactic acid,umbilical vein pH,umbilical vein lactic acid.)as well as differences in the maternal placental microbiome were analyzed.Microbial diversity indices(Shannon index,Simpson index,Richness index,Chao index,ACE index)were recorded,and the placental microbial community structure was examined.In addition,fetal cord blood levels of IGF-1 and IGFBP-3 were compared between the two groups.Results There were no statistically significant differences in maternal age,pre-pregnancy body mass index,nationality,birth history,neonatal gender,birth mode,umbilical artery pH,umbilical artery lactic acid,umbilical vein pH and umbilical vein lactic acid between the healthy group and the small sample group(P>0.05).The birth length of the small sample group was shorter than that of the healthy group,the difference was statistically significant(P<0.05).There were no statistically significant differences between the groups in terms of Shannon index,Simpson index,Richness index,Chao index,and ACE index(P>0.05).In the healthy group,the dominant placental microbiota included the phyla Proteobacteri
