摘 要:目的探究妊娠期高血压孕妇尿液N-乙酰-β-D-氨基葡萄糖苷酶(NAG)、尿液中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、尿液β2-微球蛋白(β2M)、血清肌酐与肾损伤的关系。方法回顾性选取2017年10月至2023年6月于首都医科大学附属北京康复医院建档产检的88例妊娠期高血压孕妇为研究组,88名健康孕妇为对照组。根据肌酐清除率(CCr)将妊娠期高血压孕妇分组:肾功能正常组(CCr≥80 mL/min,n=56)、肾功能受损组(CCr<80 mL/min,n=32)。分别比较研究组和对照组,肾功能正常组和肾功能受损组的尿液NAG、尿液NGAL、尿液β2M、血清肌酐水平差异;并比较肾功能正常组和肾功能受损组肾脏血流参数指标(舒张末期流速、S/D比值、搏动指数、阻力指数、收缩期峰值流速、加速时间、加速度等)差异;采用Pearson相关分析探讨尿液NAG、尿液NGAL、血清肌酐、尿液β2M水平与肾损伤程度的相关性。结果研究组的mALB、血清尿素氮、血清肌酐、尿液NAG、尿液NGAL、尿液β2M、尿液KIM-1分别为(8.31±1.23)mg/mmol、(5.16±1.12)mmol/L、(88.26±14.26)μmol/L、(18.87±3.26)U/L、(36.64±4.15)g/L、(1.01±0.28)mg/L、(9.64±1.45)ng/L,均高于对照组[(5.07±1.12)mg/mmol、(3.55±0.78)mmol/L、(71.22±16.11)μmol/L、(9.11±1.56)U/L、(13.08±3.44)g/L、(0.28±0.07)mg/L、(5.44±1.26)ng/L],差异均有统计学意义(P<0.05)。肾功能受损组尿液NAG、尿液NGAL、mALB、血清尿素氮、血清肌酐、尿液β2M、尿液KIM-1水平均高于肾功能正常组,差异均有统计学意义(P<0.05)。肾功能正常组和肾功能受损组妊娠期高血压孕妇组间舒张末期流速、S/D比值、搏动指数、阻力指数比较,差异均无统计学意义(P>0.05);肾功能受损组妊娠期高血压孕妇的收缩期峰值流速、加速度低于肾功能正常组,加速时间长于肾功能正常组,差异均有统计学意义(P<0.05)。妊娠期高血压孕妇尿液NAG、尿液NGAL、尿液β2M、血清�Objective To explore the relationship between urinary N-acetyl-β-d-glucosaminidase(NAG),urinary neutrophil gelatinase associated lipocalin(NGAL),urinary β2-microglobulin(β2M),serum creatinine and renal injury in pregnant women with gestational hypertension.Methods A total of 88 pregnant women with gestational hypertension who were registered in Beijing Rehabilitation Hospital,Capital Medical University from October 2017 to June 2023 were retrospectively selected as the study group,and 88 healthy pregnant women were selected as the control group.According to the creatinine clearance rate(CCr)of patients with gestational hypertension,they were divided into the normal renal function group(CCr≥80 mL/min,n=56)and the impaired renal function group(CCr<80 mL/min,n=32).The levels of urine NAG,urine NGAL,urine β2M and serum creatinine were compared between the study group and the control group,the normal renal function group and the impaired renal function group.The renal blood flow parameters(end-diastolic velocity,S/D ratio,pulsatility index,resistance index,peak systolic velocity,acceleration time,acceleration,etc.)were compared between the normal renal function group and the impaired renal function group.The correlation between urine NAG,urine NGAL,serum creatinine,urine β2M levels and the degree of renal injury was explored using Pearson correlation analysis.Results The mALB,serum urea nitrogen,serum creatinine,urine NAG,urine NGAL,urine β2M and urine KIM-1 in the study group were(8.31±1.23)mg/mmol,(5.16±1.12)mmol/L,(88.26±14.26)μmol/L,(18.87±3.26)U/L,(36.64±4.15)g/L,(1.01±0.28)mg/L,(9.64±1.45)ng/L,respectively,which were higher than those in the control group[(5.07±1.12)mg/mmol,(3.55±0.78)mmol/L,(71.22±16.11)μmol/L,(9.11±1.56)U/L,(13.08±3.44)g/L,(0.28±0.07)mg/L,(5.44±1.26)ng/L],and the differences were statistically significant(P<0.05).The levels of urine NAG,urine NGAL,mALB,serum urea nitrogen,serum creatinine,urine β2M and urine KIM-1 in the impaired renal function group were higher than t
