能谱与非能谱扫描技术在腹部检查中辐射剂量及图像质量的对比研究

摘  要:目的观察对比GE Revolution 256宽体CT能谱与非能谱扫描技术在腹部检查中的辐射剂量及图像质量。方法收集我院50例上腹部增强CT扫描患者,使用GE R evolution 256宽体探测器螺旋CT,采用非能谱及能谱扫描,非能谱与能谱前置及后置ASiR-V权重均为40%,图像质量均进行主观评价和客观评价,同时记录患者两种模式扫描辐射剂量值。结果非能谱竖脊肌SD值(17.76±3.01)与能谱竖脊肌SD值(20.55±3.09)、非能谱皮下脂肪SD值(13.07±3.18)与能谱皮下脂肪SD值(16.05±3.74)比较差异均有统计学意义(P<0.05)。主观评分能谱扫描(4.09±0.19)分,高于非能谱扫描的(3.95±0.15)分,差异有统计学意义(P<0.05)。非能谱CTDLvol(8.54±0.95)mGy、DLP(268.77±43.04)mGy·cm、ED(4.03±0.65)mSv与能谱CT DLvol(8.05±1.05)mGy、DLP(250.53±39.93)mGy·cm、ED(3.76±0.60)mSv比较,差异有统计学意义(P<0.05)。结论GE Revolution 256宽体能谱上腹部CT检查扫描辐射剂量低于非能谱扫描,且图像主观评价质量高于非能谱成像,故腹部能谱扫描可作为临床腹部检查常规扫描方式。Objective Exploring the comparative study of radiation dose and image quality between Revolution 256 CT energy spectrum and non energy spectrum scanning techniques in abdominal examinations.Methods A total of 50 patients with enhanced CT scans of the upper abdomen in our hospital,use GE Revolution 256 wide body detector spiral CT,and use non spectral and spectral GSI scans.The pre and post ASiR-V weights of non spectral and spectral are both 40%,and the image quality is subjectively and objectively evaluated.At the same time,record the radiation dose values of the subjects in both modes of scanning.Results The SD values of non spectral erector spinae muscle(17.76±3.01)and spectral erector spinae muscle(20.55±3.09),as well as non spectral subcutaneous fat(13.07±3.18)and spectral subcutaneous fat(16.05±3.74)were statistically significant(P<0.05).The subjective score of energy spectrum scanning(4.09±0.19)was higher than that of non energy spectrum scanning(3.95±0.15),and the difference was statistically significant(P<0.05).The non spectral CTDLvol(8.54±0.95)mGy,DLP(268.77±43.04)mGy·cm,ED(4.03±0.65)mSv and spectral CTDLvol(8.05±1.05)mGy,DLP(250.53±39.93)mGy·cm,and ED(3.76±0.60)mSv were statistically significant(P<0.05).Conclusion Revolution 256 CT upper abdominal examination:The radiation dose of energy spectrum scanning is lower than that of non energy spectrum scanning,and the subjective evaluation quality of the image is higher than that of non energy spectrum imaging.Therefore,abdominal energy spectrum scanning can be used as a routine scanning method for clinical abdominal examination instead of conventional non energy spectrum scanning mode.
