极低出生体质量早产儿血清25-(OH)D、CRP、IL-6水平与新生儿呼吸衰竭的相关性研究

摘　　要：目的分析极低出生体质量早产儿血清25-羟维生素D[25-hydroxy vitamin D,25-(OH)D]、C反应蛋白(C-reactive protein,CRP)、白细胞介素6(interleukin 6,IL-6)水平与新生儿呼吸衰竭(neonatal respiratory failure,NRF)的相关性。方法非随机选取2021年1月—2023年12月在深圳市中西医结合医院出生的85例发生NRF的极低体质量早产儿为研究对象,按新生儿呼吸窘迫评分(Downes)分为轻度组(n=43)、中度组(n=22)、重度组(n=20)。比较3组患儿血清25-(OH)D、IL-6、CRP水平;分析极低出生体质量早产儿血清25-(OH)D、IL-6、CRP水平与Downes评分的相关性。结果与轻度组比较,中度组、重度组患儿的血清25-(OH)D水平均较低,血清IL-6、CRP水平均较高,差异有统计学意义(P均<0.05)。Pearson相关性分析显示,血清25-(OH)D水平与Downes评分呈负相关,血清IL-6、CRP水平与Downes评分呈正相关(r=-0.774、0.846、0.751,P均<0.05)。结论极低出生体质量早产儿的血清25-(OH)D、CRP、IL-6与NRF存在明显相关性。提示可将25-(OH)D、CRP、IL-6作为极低出生体质量早产儿NRF的临床评估指标,也可依此探索新的治疗靶点。Objective To analyze the correlation between serum 25-hydroxy vitamin D[25-(OH)D],C-reactive protein(CRP),interleukin 6(IL-6)levels and neonatal respiratory failure(NRF)in very low birth weight premature infants.Methods A total of eighty-five very low birth weight premature infants with NRF born in Shenzhen Integrated Traditional Chinese and Western Medicine Hospital from January 2021 to December 2023 were non-randomly selected as the research objects.According to the neonatal respiratory distress score(Downes),they were divided into mild group(n=43),moderate group(n=22)and severe group(n=20).The levels of serum 25-(OH)D,IL-6 and CRP were compared among the three groups.The correlation between serum 25-(OH)D,IL-6,CRP levels and Downes score in very low birth weight premature infants was analyzed.Results Compared with the mild group,the levels of serum 25-(OH)D in the moderate group and the severe group were lower,and the levels of serum IL-6 and CRP were higher,and the differences were statistically significant(all P<0.05).Pearson correlation analysis showed that serum 25-(OH)D level was negatively correlated with Downes score,and serum IL-6 and CRP levels were positively correlated with Downes score(r=-0.774,0.846,0.751,all P<0.05).Conclusion There was a significant correlation between serum 25-(OH)D,CRP,IL-6 and NRF in very low birth weight premature infants.It is suggested that 25-(OH)D,CRP and IL-6 can be used as clinical evaluation indexes of NRF in very low birth weight premature infants,and new therapeutic targets can also be explored accordingly.
