摘　　要：目的研究超声引导下行腰丛复合坐骨神经阻滞麻醉运用于老年股骨粗隆间骨折术的效果。方法非随机选取南京鼓楼医院集团仪征医院于2019年1月—2023年10月接受手术治疗的76例股骨粗隆间骨折老年患者为研究对象。根据麻醉方式分为两组,每组38例。对照组行蛛网膜下腔阻滞麻醉复合硬膜外麻醉,观察组行超声引导腰丛复合坐骨神经麻醉。比较两组麻醉效果。结果相较于对照组,观察组的神经阻滞起效时间更短,且维持时间更长,疼痛程度更低,差异有统计学意义(P均<0.05)。在不良反应方面,对照组总发生率为21.05%(8/38),高于观察组的5.26%(2/38),差异有统计学意义(χ^(2)=4.146,P<0.05)。结论在老年股骨粗隆间骨折术中,患者行超声引导腰丛复合坐骨神经麻醉的麻醉阻滞效果较好,且患者术后疼痛感较轻,不良反应少。Objective To study the effect of ultrasound-guided lumbar plexus combined with sciatic nerve block anesthesia in elderly patients with intertrochanteric femoral fractures.Methods A total of seventy-six elderly patients with intertrochanteric femoral fractures who received surgical treatment in Yizheng Hospital of Nanjing Gulou Hospital Group from January 2019 to October 2023 were non-randomly selected as the study objects.They were divided into two groups according to the way of anesthesia,with thirty-eight cases in each group.The control group received subarachnoid block anesthesia combined with epidural anesthesia,and the observation group received ultrasound guided lumbar plexus combined with sciatic nerve anesthesia.The anesthetic effect of the two groups was compared.Results Compared with the control group,the onset time of nerve block in the observation group was shorter,the maintenance time was longer,and the pain degree was lower,the differences were statistically significant(all P<0.05).In terms of adverse reactions,the total incidence of the control group was 21.05%(8/38),which was higher than 5.26%(2/38)of the observation group,the difference was statistically significant(χ^(2)=4.146,P<0.05).Conclusion In elderly patients with intertrochanteric fracture,ultrasound guided lumbar plexus combined with sciatic nerve anesthesia has better anesthesia block effect,and the postoperative pain is less,and the adverse reactions are less.
