呼吸道病原菌基因检测法在诊断儿童肺炎链球菌感染中的临床意义

摘  要:目的分析儿童肺炎链球菌感染中采用呼吸道病原菌基因检测法的价值。方法非随机选取2022年1月—2023年12月平度市人民医院收治的153例疑似儿童肺炎链球菌感染患儿为研究对象。所有患儿均接受传统病原学培养和呼吸道病原菌基因检测,以传统病原学培养结果为金标准,分析呼吸道病原菌基因检测的诊断效能。结果传统病原学培养结果显示,153例患儿中有100例阳性、53例阴性。呼吸道病原菌基因检测的敏感度为96.00%(96/100),特异度为96.23%(51/53),准确度为96.08%(147/153),Kappa值为0.914。且ROC曲线显示,线下面积(area under the curve,AUC)为0.861,95%CI(0.796~0.921)。结论在儿童肺炎链球菌感染的诊断过程中,呼吸道病原菌基因检测的诊断效能较高,与传统病原学培养具有较高的一致性。Objective To analyze the value of using genetic test for respiratory pathogens in streptococcus pneumoniae infections in children.Methods A total of 153 children with suspected childhood streptococcus pneumoniae infections admitted to Pingdu People's Hospital from January 2022 to December 2023 were non-randomly selected as study subjects.All children received traditional pathogenic culture and respiratory pathogen genetic testing,and the diagnostic efficacy of respiratory pathogen genetic testing was analyzed using traditional pathogenic culture results as the gold standard.Results The results of traditional pathogenicity culture showed 100 positive and 53 negative cases out of 153 children.The sensitivity of the genetic test for respiratory pathogens was 96.00%(96/100),the specificity was 96.23%(51/53),the accuracy was 96.08%(147/153),and the Kappa value was 0.914.ROC curve showed that the area under the curve(AUC)was 0.861,95%CI(0.796~0.921).Conclusion In the diagnostic process of streptococcus pneumoniae infection in children,the diagnostic efficacy of genetic testing of respiratory pathogens is high,with a high degree of concordance with traditional pathogenetic cultures.
