摘 要:目的研究血涂片细胞形态学检测与全自动血液细胞分析仪检测联合应用在缺铁性贫血检验中的临床效果。方法非随机选取新疆生产建设兵团第五师医院于2023年5月—2024年1月期间诊治的96例疑似缺铁性贫血患者为研究对象,所有患者均进行细胞形态学检测和全自动血液细胞分析仪检测,以临床综合诊断结果作为金标准,比较两种检测方式的单一及联合诊断的诊断效能。结果96例疑似缺铁性贫血患者中,临床综合诊断阳性70例,阴性26例。血涂片细胞形态学检测的特异度为61.54%(16/26)、敏感度为71.43%(50/70)、准确度为68.75%(66/96);全自动血液细胞分析仪检测的特异度为65.38%(17/26)、敏感度为75.71%(53/70)、准确度为72.92%(70/96);两种方法联合检测的特异度为92.31%(24/26)、敏感度为98.57%(69/70)、准确度为96.88%(93/96),联合检测的诊断效能高于单一检测结果,差异有统计学意义(χ^(2)=7.428、9.068、27.156,P均<0.05)。结论在对疑似缺铁性贫血患者的诊断中,联合应用血涂片细胞形态学检测与全自动血液细胞分析仪能够提高疾病检测的特异度、敏感度和准确度,为临床诊断和治疗提供更可靠的依据。Objective To study the clinical effect of combined application of blood smear cell morphology detection and automatic blood cell analyzer detection in Iron deficiency anemia test.Methods A total of ninety-six patients with suspected Iron deficiency anemia diagnosed and treated in the Fifth Division Hospital of Xinjiang Production and Construction Corps from May 2023 to January 2024 were non-randomly selected as the research objects.All patients underwent cell morphology detection and automatic blood cell analyzer detection,with clinical comprehensive diagnosis results as the gold standard.Compare the diagnostic performance of the two detection methods individually and in combination.Results Among the 96 suspected Iron deficiency anemia patients,70 were clinically diagnosed as positive,and 26 as negative.The specificity,sensitivity,and accuracy of blood smear cytomorphological detection were 61.54%(16/26),71.43%(50/70),and 68.75%(66/96),respectively.For the automated blood cell analyzer,the specificity,sensitivity,and accuracy were 65.38%(17/26),75.71%(53/70),and 72.92%(70/96),combined detection of the two methods achieved a specificity of 92.31%(24/26),sensitivity of 98.57%(69/70),and accuracy of 96.88%(93/96),the differences in diagnostic performance for the combined detection method were statistically significant(χ^(2)=7.428,9.068,27.156,all P<0.05).Conclusion In the diagnosis of suspected Iron deficiency anemia,the combined application of blood smear cytomorphological detection and automated blood cell analyzer can improve the specificity,sensitivity,and accuracy of disease detection.This provides more reliable support for clinical diagnosis and treatment.
