血脂水平和凝血指标与2型糖尿病肾脏病临床病理分期的相关性研究

摘　　要：目的分析血脂水平和凝血指标与2型糖尿病肾脏病临床病理分期的相关性。方法回顾性选取2022年1月—2023年12月天水市第一人民医院收治的160例2型糖尿病患者的临床资料,按照是否并发肾脏病分为单纯2型糖尿病组(n=102)和2型糖尿病肾脏病组(n=58),选取同期的健康体检者作为对照组(n=80)。2型糖尿病肾脏病组依据临床分期分为Ⅰ期(n=11)、Ⅱ期(n=13)、Ⅲ期(n=19)、Ⅳ期(n=9)、V期(n=6)。比较各组的血脂、凝血四项水平,分析其与2型糖尿病肾脏病临床病理分期的相关性。结果单纯2型糖尿病组、2型糖尿病肾脏病组的甘油三酯、凝血酶时间、纤维蛋白原均高于对照组,2型糖尿病肾脏病组高于单纯2型糖尿病组,差异有统计学意义(P均<0.05)。2型糖尿病肾脏病不同临床分期血脂、凝血指标比较,差异有统计学意义(P均<0.05)。甘油三酯、凝血酶时间、纤维蛋白原均与2型糖尿病肾脏病临床病理分期之间存在正相关(r=0.872、0.609、0.628,P均<0.05)。结论2型糖尿病肾脏病病情严重程度与血脂异常、凝血功能紊乱存在相关性,临床可据此及早进行干预。Objective To analyze the correlation between blood lipid level and coagulation index and clinicopathological stage of type 2 diabetic kidney disease.Methods The clinical data of 160 patients with type 2 diabetes admitted to Tianshui First People's Hospital from January 2022 to December 2023 were retrospectively selected,and they were divided into type 2 diabetes group(n=102)and type 2 diabetes group(n=58)according to whether they were complicated with kidney disease,and healthy subjects in the same period were selected as the control group(n=80).Type 2 diabetic nephropathy group was divided into stage I(n=11),stageⅡ(n=13),stageⅢ(n=19),stageⅣ(n=9)and stage V(n=6)according to clinical stages.The levels of blood lipids and coagulation in all groups were compared,and the correlation with clinicopathological stage of type 2 diabetes kidney disease was analyzed.Results The triglyceride,thrombin time and fibrinogen of type 2 diabetes group and type 2 diabetes kidney disease group were higher than those of control group,and the differences were statistically significant(all P<0.05).There were significant differences in blood lipids and coagulation indexes in different clinical stages of type 2 diabetes kidney disease(all P<0.05).Triglyceride,thrombin time and fibrinogen were positively correlated with the clinicopathologic stage of type 2 diabetic kidney disease(r=0.872,0.609,0.628,all P<0.05).Conclusion The severity of type 2 diabetes renal disease is related to dyslipidemia and coagulation disorder,which can be used for early intervention.
