摘  要:目的分析学龄期单纯性肥胖儿童维生素D水平与脂代谢、胰岛素抵抗的相关性。方法非随机选取2021年5—11月红河州滇南中心医院收治的60例学龄期单纯性肥胖儿童为研究对象,根据维生素D水平分为2组[维生素D<50 nmol/L组(n=52)、维生素D≥50 nmol/L组(n=8)]。检测两组单纯性肥胖儿童总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、空腹血糖(fasting blood glucose,FBG)、空腹胰岛素(fasting insulin,FINS)、胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)含量,并计算胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOMA-IR)。根据检测结果分组,分析不同维生素D水平儿童的脂代谢和胰岛素水平变化,并采用Pearson相关分析法探讨维生素D水平与脂代谢及胰岛素抵抗的相关性。结果两组患儿HDL-C、TC、TG、LDL-C、HOMA-IR、IGF-1水平比较,差异有统计学意义(P均<0.05)。Pearson相关性分析结果显示,维生素D水平与HDL-C、HOMA-IR呈正相关(r=0.342、0.314,P<0.05),而与TC、TG、LDL-C及IGF-1呈负相关(r=-0.264、-0.316、-0.294、-0.291,P均<0.05)。结论学龄期单纯性肥胖儿童维生素D水平与脂代谢、胰岛素抵抗密切相关,因此需要采取合理措施对血脂代谢、胰岛素抵抗情况进行干预,减轻对儿童生长发育的影响。Objective To analyze the correlation between vitamin D levels,lipid metabolism,and insulin resistance in school-age children with simple obesity.Methods From May to November 2021,sixty school-aged children with simple obesity admitted to Honghe Prefecture Diannan Central Hospital were non-randomly selected as the study subjects.Based on their vitamin D levels,The participants were divided into two groups based on vitamin D levels[vitamin D<50 nmol/L group(n=52)and vitamin D≥50 nmol/L group(n=8)].The levels of total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),fasting blood glucose(FBG),fasting insulin(FINS),and insulin-like growth factor-1(IGF-1)were measured in both groups,and the homeostasis model assessment of insulin resistance(HOMA-IR)was calculated.Based on the test results,the changes in lipid metabolism and insulin levels in children with different vitamin D levels were analyzed.Pearson correlation analysis was used to explore the relationship between vitamin D levels,lipid metabolism,and insulin resistance.Results Compare the HDL-C,TC,TG,LDL-C,HOMA-IR,and IGF-1 levels between the two groups,the differences were statistically significant(all P<0.05).Pearson correlation analysis showed that vitamin D level was positively correlated with HDL-C,HOMA-IR(r=0.342,0.314,P<0.05),and negatively correlated with TC,TG,LDL-C and IGF-1(r=-0.264,-0.316,-0.294,-0.291,all P<0.05).Conclusion Vitamin D levels in school-aged children with simple obesity are closely associated with lipid metabolism and insulin resistance.Therefore,appropriate measures should be taken to intervene in lipid metabolism and insulin resistance to reduce their impact on children's growth and development.
