左甲状腺素钠片应用于妊娠期亚临床甲减治疗中的临床效果分析

摘　　要：目的分析在妊娠期亚临床甲状腺功能减退症(简称甲减)的临床治疗过程中应用左甲状腺素钠片的效果。方法回顾性选取2021年2月—2023年2月麻城市人民医院产科收治的90例妊娠期亚临床甲减产妇的临床资料,根据治疗方法不同分成参照组(常规方式进行治疗)和研究组(左甲状腺素钠片治疗),每组45例。比较两组临床疗效、甲状腺激素、血清同型半胱氨酸(homocysteine,Hcy)、叶酸、孕妇不良结局、新生儿不良结局。结果研究组总有效率高于参照组,差异无统计学意义(P>0.05);治疗后,研究组甲状腺激素、孕妇不良结局、新生儿不良结局优于参照组,差异有统计学意义(P均<0.05);研究组血清Hcy(3.35±1.18)μmol/L、叶酸(12.49±2.55)ng/mL均优于参照组的(4.89±1.49)μmol/L和(9.98±3.78)ng/mL,差异有统计学意义(t=5.435、3.693,P均<0.05)。结论对妊娠期亚临床甲减开展左甲状腺素钠片治疗可及时纠正甲状腺激素,避免不良妊娠以及新生儿结局。Objective To analyze the effectiveness of levothyroxine sodium tablets in the clinical treatment of subclinical hypothyroidism during pregnancy.Methods Clinical date of ninety pregnant women with subclinical hypothyroidism admitted to the Obstetrics Department of Macheng People's Hospital from February 2021 to February 2023 were retrospectively selected and divided into the reference group(conventional treatment)and the study group(levothyroxine sodium tablet treatment),with forty-five groups each according to different treatment methods.The clinical efficacy,thyroid hormone indexes,serum homocysteine(Hcy),folate,adverse outcomes of pregnant women,neonatal adverse outcomes were compared.Results The total effective value of the study group was higher than that of the reference group,and the difference was not statistically significant(P>0.05).After treatment,the thyroid hormone indexes,adverse outcome of pregnant women,neonatal adverse outcome in the study group were better than those in the reference group,and the differences were statistically significant(all P<0.05).After treatment,the serum Hcy(3.35±1.18)μmol/L and folic acid(12.49±2.55)ng/mL in the study group were better than(4.89±1.49)μmol/L and(9.98±3.78)ng/mL in the reference group,and the differences were statistically significant(t=5.435,3.693,both P<0.05).Conclusion When levothyroxine sodium tablets are used to treat subclinical hypothyroidism in pregnancy,thyroid hormones can be corrected in time,which can avoid adverse pregnancy and neonatal outcome.
