摘 要:目的探讨小剂量纳布啡预给药对罗库溴铵缩肢反应的影响。方法非随机选取2023年1—12月汕头大学医学院第二附属医院收治的60例择期气管插管全麻手术患者为研究对象。根据麻醉诱导方式不同分为生理盐水组及纳布啡组,每组30例。纳布啡组在麻醉诱导前3 min静脉注射0.1 mg/kg的纳布啡,生理盐水组在麻醉诱导前3 min静脉注射2 mL生理盐水。比较两组患者注射罗库溴铵后10 s内的缩肢反应及入室后安静状态下(T0)及给药3 min后(T1)心率、平均动脉压、外周血氧饱和度。结果纳布啡组与生理盐水组在T0时心率、平均动脉压及血氧饱和度等指标比较,差异无统计学意义(P均>0.05)。纳布啡组T1时心率为(77.29±12.76)次/min,慢于生理盐水组的(92.74±8.90)次/min,平均动脉压为(108.15±9.82)mmHg,低于生理盐水组的(125.73±13.52)mmHg,差异有统计学意义(t=6.439、5.179,P均<0.05)。纳布啡组缩肢反应总发生率低于生理盐水组,差异有统计学意义(P<0.05)。结论0.1 mg/kg纳布啡在气管插管前可降低罗库溴铵诱导缩肢反应,维持循环功能稳定。Objective To investigate the effect of low-dose nalbuphine pretreatment on rocuronium contraction re-sponse.Methods From January to December 2023,a total of sixty patients undergoing elective tracheal intubation sur-gery under general anesthesia in the Second Affiliated Hospital of Shantou University Medical College were non-randomly selected as the research objects.They were divided into normal saline group and nalbuphine group accord-ing to different anesthesia induction methods,with thirty cases in each group.In the nalbuphine group,0.1 mg/kg of nalbuphine was injected intravenously 3 min before anesthesia induction,and in the normal saline group,2 mL of nor-mal saline was injected intravenously 3 min before anesthesia induction.The limb contraction response within 10 s af-ter injection of rocuronium,heart rate,mean arterial pressure and peripheral blood oxygen saturation were compared between the two groups under quiet state(T0)and 3 min after administration(T1).Results There were no significant differences in heart rate,mean arterial pressure and blood oxygen saturation between nalbuphine group and saline group at T0(all P>0.05).At T1,the heart rate in nalbuphine group was(77.29±12.76)beats/min,which was slower than(92.74±8.90)beats/min in saline group,and the mean arterial pressure in nalbuphine group was(108.15±9.82)mmHg,which was lower than(125.73±13.52)mmHg in saline group,the differences were statistically significant(t=6.439,5.179,both P<0.05).The total incidence of limb withdrawal reaction in nalbuphine group was lower than that in normal saline group,and the difference was statistically significant(P<0.05).Conclusion 0.1mg/kg nalbuphine can reduce rocuronium-induced limb contraction reaction before tracheal intubation and maintain the stability of circula-tory function.
