分析ENA抗体检验和血清体液免疫检验诊断系统性红斑狼疮的效果

摘  要:目的在系统性红斑狼疮(systemic lupus erythematosus,SLE)的临床诊断中,辅助应用可提取性核抗原(extractable nuclear antigen,ENA)抗体检验和血清体液免疫检测,旨在探索其在SLE诊断中的作用和影响。方法非随机选取淄博市中医医院于2023年1—12月诊治的50例SLE患者以及同时间段内到该医院进行健康体检的30名受检者为研究对象。50例患者根据SLE病情活动程度评分分为活动期组(≥5分,n=21)与非活动期组(<5分,n=29)。所有研究对象均接受ENA抗体及血清体液免疫检测,并比较不同组别在抗体阳性率、血清体液免疫指标的差异。通过评估ENA抗体、血清体液免疫指标和联合检验的诊断效能,包括敏感度、特异度和准确度,分析其在SLE诊断中的应用价值。结果在抗双联DNA阳性率、抗干燥综合征A阳性率、抗干燥综合征B阳性率、抗Sm阳性率、抗U1小核糖核蛋白抗体阳性率方面,活动期组和非活动期组相较于对照组均更高,差异有统计学意义(P均<0.05)。相较于对照组和非活动期组,活动期组的血清免疫球蛋白M、血清免疫球蛋白A及血清免疫球蛋白G更高,补体3与补体4水平更低,差异有统计学意义(P均<0.05)。在诊断效能方面,血清体液免疫指标和ENA抗体联合检测的准确度为95.00%(76/80),高于ENA抗体的77.50%(62/80)和血清体液免疫指标的63.75%(51/80),差异有统计学意义(χ^(2)=23.455,P<0.05);此外,联合检测的敏感度和特异度均高于两者单一检测,差异有统计学意义(P均<0.05)。结论ENA与血清体液免疫指标联合应用于SLE的诊断效果较好,有利于明确病情发展状况。Objective In the clinical diagnosis of systemic lupus erythematosus(SLE),extractable nuclear antigen(ENA)antibody test and serum humoral immunity test are used to explore its role and influence in the diagnosis of SLE.Methods Fifty SLE patients diagnosed and treated in Zibo Traditional Chinese Medicine Hospital from January to December 2023 and thrity subjects who went to the hospital for physical examination during the same period were non-randomly selected as the research objects.According to the SLE disease activity score,fifty patients were divided into active group(≥5,n=21)and inactive group(<5,n=29).All subjects were tested for ENA antibody and serum hu-moral immunity,and the differences in antibody positive rate,immunoglobulin and complement levels between differ-ent groups were compared.By evaluating the diagnostic efficacy of ENA antibody,serum humoral immune index and combined test,including sensitivity,specificity and accuracy,its application value in the diagnosis of SLE was ana-lyzed.Results In terms of anti-double DNA positive rate,anti-desiccation syndrome A positive rate,anti-desiccation syndrome B positive rate,anti-Sm positive rate,and anti-U1 small nuclear ribonucleoprotein antibody positive rate,the active group and the inactive group were higher than the control group,and the differences were statistically sig-nificant(all P<0.05).Compared with the control group and the inactive group,the serum immunoglobulin M,serum im-munoglobulin A and serum immunoglobulin G in the active group were higher,and the levels of complement 3 and complement 4 were lower,the differences were statistically significant(all P<0.05).In terms of diagnostic efficacy,the accuracy of combined detection of serum humoral immune index and ENA antibody was 95.00%(76/80),which were higher than 77.50%(62/80)of ENA antibody and 63.75%(51/80)of serum humoral immune index,and the difference was statistically significant(χ^(2)=23.455,P<0.05).In addition,the sensitivity and specificity of combined detection were higher than thos
