色素内镜在胃癌临床诊断中的应用价值

摘　　要：目的分析色素内镜诊断胃癌的效能。方法目的选取2018年8月—2023年8月遵义市第一人民医院(遵义医科大学第三附属医院)收治的113例疑似胃癌患者为研究对象,均进行常规内镜、色素内镜检查,以胃黏膜病理活检结果为“金标准”,分析色素内镜对胃癌的诊断效能。结果113例疑似胃癌的患者中,“金标准”共检出65例阳性;色素内镜共检出64例阳性,常规内镜共检出56例阳性;以病理检查结果为“金标准”,色素内镜诊断胃癌的灵敏度为96.92%(63/65)、准确度为97.35%(110/113)、阴性预测值为95.92%(47/49),高于常规内镜的83.08%(54/65)、88.50%(100/113)、80.70%(46/57),差异有统计学意义(χ^(2)=6.923、6.726、5.670,P均<0.05);一致性分析结果显示,色素内镜与“金标准”的一致性较好(kappa=0.946,P<0.05)。结论色素内镜在胃癌诊断中表现出较高的效能,其灵敏度和准确度均处在较高水平。Objective To analyze the efficacy of chromoendoscopy in the diagnosis of gastric cancer.Methods A total of 113 patients with suspected gastric cancer admitted to The First People's Hospital of Zunyi City(The Third Affili-ated Hospital of Zunyi Medical University)from August 2018 to August 2023 were purpose selected as the research objects.All patients underwent routine endoscopy and chromoendoscopy.The results of gastric mucosal pathological biopsy were used as the"gold standard"to analyze the diagnostic efficacy of chromoendoscopy in gastric cancer.Re-sults Among 113 patients with suspected gastric cancer,65 cases were positive in the"gold standard".A total of 64 positive cases were detected by chromoendoscopy,and 56 positive cases were detected by conventional endoscopy.Taking the pathological examination results as the"gold standard",the sensitivity,accuracy and negative predictive value of chromoendoscopy in the diagnosis of gastric cancer were 96.92%(63/65),97.35%(110/113)and 95.92%(47/49),which were higher than 83.08%(54/65),88.50%(100/113)and 80.70%(46/57)of conventional endoscopy,and the differences were statistically significant(χ^(2)=6.923,6.726,5.670,all P<0.05).Consistency analysis showed that the consistency between pigment endoscopy and"gold standard"was good(kappa=0.946,P<0.05).Conclusion Pigment endoscopy shows high efficacy in the diagnosis of gastric cancer,and its sensitivity and accuracy are at a high level.
