超早期小骨窗微创手术对高血压脑出血神经功能的影响

摘　　要：目的分析超早期小骨窗微创手术对高血压脑出血患者神经功能的影响。方法非随机选取2022年10月—2024年2月青岛即墨区人民医院收治的86例高血压脑出血患者为研究对象,按手术方式分组。参照组43例行常规大骨瓣开颅血肿清除术,观察组43例行超早期小骨窗微创手术,对比两组的临床效果。结果与参照组相比,观察组围术期指标更优,差异有统计学意义(P均<0.05)。观察组血肿清除率为93.02%(40/43),高于参照组的的76.74%(33/43),差异有统计学意义(χ^(2)=4.440,P<0.05)。观察组神经功能优于参照组,差异有统计学意义(P<0.05)。与参照组相比,观察组的并发症发生情况和预后情况更优,差异有统计学意义(P均<0.05)。结论高血压脑出血患者行超早期小骨窗微创手术治疗,可以有效改善神经功能,缩短手术时间和住院时间,减少术中出血量,血肿清除率和安全性较高。Objective To analyze the effect of ultra-early small bone window minimally invasive surgery on the neuro-logical function of hypertensive cerebral hemorrhage patients.Methods A total of eighty-six cases of hypertensive in-tracerebral hemorrhage treated in Qingdao Jimo District People's Hospital from October 2022 to February 2024 were non-randomly selected,and grouped by operation method.The reference group(n=43)underwent conventional large bone flap craniotomy hematoma removal,and the observation group(n=43)underwent ultra-early small bone window minimally invasive surgery,and the clinical effects of the two groups were compared.Results Compared with the refer-ence group,the perioperative indicators in the observation group were better,the differences were statistically signifi-cant(all P<0.05).The clearance rate of hematoma in the observation group was 93.02%(40/43),higher than 76.74%(33/43)in the reference group,and the difference was statistically significant(χ^(2)=4.440,P<0.05).Compared with the reference group,the neurological function,occurrence of complications and prognosis of the observation group was bet-ter,and the difference was statistically significant(both P<0.05).Conclusion Ultra-early minimally invasive surgery with small bone window can effectively improve nerve function,shorten operation time and hospital stay,reduce intra-operative blood loss,and have high hematoma clearance rate and safety.
