米索前列醇联合子宫颈扩张球囊引产足月妊娠期糖尿病初产妇的疗效分析

摘　　要：目的探讨米索前列醇联合子宫颈扩张球囊引产足月妊娠期糖尿病初产妇的疗效及安全性。方法选取2021年1月至2024年5月黄山市人民医院产科收治的102例引产治疗的足月妊娠期糖尿病初产妇,按照使用不同促宫颈成熟方式,分为观察组和对照组,每组各51例。观察组采用米索前列醇阴道片联合子宫颈扩张球囊引产,对照组单独采用子宫颈扩张球囊引产。比较两组分娩结局、促宫颈成熟有效率、产程时间、产时及产后出血量、感染指标、血糖变化等。结果观察组促宫颈成熟有效率(94.11%比80.40%)及阴道分娩率(70.59%比49.02%)高于对照组(P<0.05)。观察组产时产后出血量、产时尿潴留、产后首次自主排尿时间、胎儿宫内窘迫、新生儿低血糖、新生儿高胆红素血症、第一产程及总产程时间均低于对照组(P<0.05)。结论米索前列醇阴道片联合子宫颈扩张球囊可提高促宫颈成熟总有效率和阴道分娩率,缩短第一产程和总产程时间,减少产时尿潴留,促进产后自主排尿,减少产时产后出血量,降低母儿并发症发生率。Objective To investigate the efficacy and safety of misoprostol combined with cervical dilatation balloon induction of labor in full-term primipara with gestational diabetes mellitus.Methods A total of 102 full-term primiparas with gestational diabetes admitted to the Department of Obstetrics of the People's Hospital of Huangshan from January 2021 to May 2024 were selected for analysis.According to the use of different methods to promote cervical ripening,they were divided into observation group and control group,with 51 cases in each group.The observation group was treated with misoprostol vaginal tablets combined with cervical dilatation balloon induction,and the control group was treated with cervical dilatation balloon induction.The delivery outcome,the effective rate of cervical ripening,the duration of labor,the amount of bleeding during labor and postpartum,the infection index,and the change of blood glucose were compared between the two groups.Results The effective rate of cervical ripening(94.11%vs.80.40%)and vaginal delivery rate(70.59%vs.49.02%)in the observation group were higher than those in the control group(P<0.05).The amount of postpartum hemorrhage during labor,urinary retention during labor,the time of the first spontaneous urination after delivery,fetal intrauterine distress,neonatal hypoglycemia,neonatal hyperbilirubinemia,the first stage of labor and the total stage of labor in the observation group were lower than those in the control group(P<0.05).Conclusion Misoprostol vaginal tablet combined with cervical dilatation balloon can improve the total effective rate of cervical ripening and vaginal delivery rate,shorten the first stage of labor and the total labor time,reduce urinary retention during labor,promote postpartum spontaneous urination,reduce postpartum hemorrhage during labor,and reduce the incidence of maternal and child complications.
