摘　　要：目的探讨自拟益气养阴活血通络方联合针灸治疗气阴两虚兼瘀型2型糖尿病合并勃起功能障碍(T2DM合并ED)疗效及对勃起质量量表(EQS)评分、勃起硬度(EHS)评分的影响。方法选取2019年6月至2020年12月唐山市中医医院气阴两虚兼瘀型T2DM合并ED的男性病人120例进行前瞻性研究,按随机数字表法分为A组(40例)、B组(40例)和C组(40例)。A组给予西医基础治疗;B组在A组治疗的基础上联合自拟益气养阴活血通络方治疗;C组在B组治疗的基础上配合针灸治疗。比较三组治疗前后国际勃起功能指数-5(IIEF-15)评分、中医证候积分、EQS评分、EHS评分、阴茎血流动力学[阴茎血管收缩期峰值流速(PSV)、舒张期峰值流速(EDV)和阻力指数(RI)]及血清性激素[睾酮(T)、雌二醇(E_(2))]、血管内皮功能[内皮素-1(ET-1)、一氧化氮(NO)、血管紧张素Ⅱ(AngⅡ)]水平。观察三组临床疗效及不良反应情况。结果C组总体疗效(治愈18例、显效15例、有效5例、无效2例)均优于A组(治愈8例、显效13例、有效7例、无效12例)、B组(治愈9例、显效15例、有效8例、无8效例)(P<0.05)。治疗后C组、B组中医证候积分均低于A组,且C组中医证候积分低于B组(P<0.05)。治疗后C组、B组IIEF-5评分、EQS评分、EHS评分均高于A组,且C组IIEF-5评分、EQS评分、EHS评分高于B组(P<0.05)。治疗后C组、B组T水平均高于A组,且C组T水平高于B组(P<0.05);治疗后C组、B组E2水平均低于A组,且C组E2水平低于B组(P<0.05)。治疗后C组、B组PSV、RI水平高于A组,且C组PSV、RI水平高于B组(P<0.05);治疗后C组EDV均低于B组、A组(P<0.05),但B组、A组比较差异无统计学意义(P>0.05)。治疗后C组、B组AngⅡ水平低于A组,且C组AngⅡ水平低于B组(P<0.05);治疗后C组ET-1水平均低于B组、A组(P<0.05),但B组、A组比较差异无统计学意义(P>0.05);治疗后C组、B组NO水平高于A组,且C组NO水平高于B组(P<0.05)。结论自拟益气�Objective To investigate the curative effect of self-made Yiqi Yangyin Huoxue Tongluo recipe combined with acupuncture on type 2 diabetes mellitus complicated with erectile dysfunction(T2DM and ED)caused by qi yin deficiency and blood stasis,and its influence on the score of erectile quality scale(EQS)and erectile hardness score(EHS).Methods One hundred and twenty male patients with T2DM and ED of type of deficiency of both qi and yin with blood stasis in Tangshan Traditional Chinese Medicine Hospital from June 2019 to December 2020 were selected for prospective study.They were assigned into group A(40 cases),group B(40 cases)and group C(40 cases)according to random number table method.Group A was given basic Western medicine treatment;Group B was treated on the basis of group A combined with self-prepared Yiqi Yangyin Huoxue Tongluo prescription;Group C was treated with acupuncture and moxibustion on the basis of group B.The international erectile function index-5(IIEF-15)scores,traditional chinese medicine(TCM)syndrome scores,EQS scores,EHS scores,penis hemodynamics[peak systolic velocity(PSV),peak diastolic velocity(EDV)and resistance index(RI)],the levels of serum sex hormones[testosterone(T),estradiol(E_(2))]and vascular endothelial function[endothelin-1(ET-1),nitric oxide(NO)and angiotensinⅡ(AngⅡ)]were compared among the three groups before and after treatment.The clinical efficacy and adverse reactions of the three groups were observed.Results The overall efficacy of group C(cured 18 cases,effective 15 cases,effective 5 cases,ineffective 2 cases)was better than that of group A(cured 8 cases,effective 13 cases,effective 7 cases,ineffective 12 cases)and group B(cured 9 cases,obvious 15 cases,effective 8 cases,no effect 8 cases)(P<0.05).After treatment,the TCM syndrome score of group C and group B was lower than that of group A,and the TCM syndrome score of group C was lower than that of group B(P<0.05).After treatment,IIEF-5 scores,EQS scores and EHS scores of groups C and B were higher than those of gro
