摘 要:目的分析D-二聚体(D-Dimer,D-D)、心肌肌钙蛋白I(troponin I cardiac muscle,cTnI)、肌红蛋白(myoglobin,MYO)及心肌酶谱检测在急性心肌梗死(acute myocardial infarction,AMI)诊断中的价值。方法非随机选取2022年2月—2024年3月徐州市铜山区中医院收治的150例AMI患者和同期的150名健康体检人群为研究对象,分别设为观察组和对照组。比较对照组和观察组肌酸激酶(creatine kinase,CK)、乳酸脱氢酶(lactate dehydrogenase,LDH)、谷草转氨酶(glutamic oxaloacetic transaminase,AST)、D-D、cTnI、MYO的水平以及CK、LDH、AST、D-D、cTnI、MYO单一及联合检测AMI的诊断效能。结果观察组CK、LDH、AST、D-D、cTnI、MYO水平均高于对照组,差异有统计学意义(P均<0.05)。通过ROC曲线分析,CK、LDH、AST、D-D、cTnI、MYO联合检测AMI的灵敏度、特异度高于单一检测。联合检测AMI的AUC为0.975,高于CK(0.751)、LDH(0.760)、AST(0.774)、D-D(0.900)、cTnI(0.858)、MYO(0.841)单一检测。结论CK、LDH、AST、D-D、cTnI、MYO水平联合检测的方式在评估AMI方面具有较高的诊断效能。Objective To analyze the diagnostic value of D-Dimer(D-D),cardiac troponin I(cTnI),myoglobin(MYO),and myocardial enzyme spectrum in acute myocardial infarction(AMI).Methods From February 2022 to March 2024,150 AMI patients and 150 healthy people in the same period admitted to Tongshan District Hospital of Traditional Chinese Medicine were non-randomly selected as the observation group and the control group.The levels of creatine kinase(CK),lactate dehydrogenase(LDH),glutamic oxaloacetic transaminase(AST),D-D,cTnI,MYO and the diagnostic efficacy of CK,LDH,AST,D-D,cTnI,MYO single and combined detection of AMI were compared between the control group and the observation group.Results The levels of CK,LDH,AST,D-D,cTnI,and MYO in the observation group were higher than control group,the differences were statistically significant(all P<0.05).ROC curve analysis showed that the combined detection of CK,LDH,AST,D-D,cTnI,and MYO had higher sensitivity and specificity for diagnosing AMI compared to single marker detection.The AUC for combined detection was 0.975,which was greater than that for CK(0.751),LDH(0.760),AST(0.774),D-D(0.900),cTnI(0.858),or MYO(0.841)alone.Conclusion The combined detection of CK,LDH,AST,D-D,cTnI,and MYO levels offers high diagnostic efficiency in evaluating AMI.
