探究膝关节骨性关节炎富血小板血浆治疗后疗效不佳的影响因素

摘　　要：目的探究膝关节骨性关节炎患者进行富血小板血浆治疗的效果,分析疗效的影响因素。方法回顾性选取2021年1月—2023年9月柳州市工人医院收治的36例膝关节骨性关节炎患者的临床资料,均完成富血小板血浆治疗,根据治疗效果分为疗效良好组(26例)和疗效不佳组(10例),对疗效不佳展开影响因素分析。结果两组年龄、治疗前K-L分级、治疗前Lysholm评分、康复训练依从性比较,差异有统计学意义(P均<0.05)。年龄>60岁、治疗前K-L分级3~4级、治疗前Lysholm评分<60分、康复训练依从性差是疗效不佳的独立危险因素(OR=3.245、3.762、3.981、3.982,P均<0.05)。结论富血小板血浆可在膝关节骨性关节炎治疗中发挥一定作用,但疗效会受年龄、K-L分级、Lysholm评分与康复训练依从性影响,从这几方面着手干预可提高临床疗效,减轻疾病对患者身心健康造成的不良影响。Objective To explore the effect of platelet-rich plasma therapy after the development of knee OA and analyze the influencing factors of efficacy.Methods The clinical data of thirty-six patients with knee osteoarthritis admitted to Liuzhou Workers Hospital from January 2021 to September 2023 were retrospectively selected,and all completed platelet-rich plasma treatment.According to the treatment effect,they were divided into good efficacy group(twenty-six cases)and poor efficacy group(ten cases).The factors of poor efficacy were analyzed.Results There were statistical significant differences in age,pre-treatment K-L grade,pre-treatment Lysholm score,and rehabilitation training compliance between two groups(all P<0.05).Age>60 years,pre-treatment K-L grade 3-4,pre-treatment Lysholm score<60,poor compliance to rehabilitation training were independent risk factors for poor efficacy(OR=3.245,3.762,3.981,3.982,all P<0.05).Conclusion Platelet-rich plasma can play a certain role in the treatment of knee osteoarthritis,but the efficacy will be affected by age,K-L grade,Lysholm score and rehabilitation training compliance.The intervention from these aspects can improve the clinical efficacy and reduce the adverse effects of the disease on the physical and mental health of patients.
