尿常规在尿路感染快速诊断中的应用价值探讨

摘　　要：目的探讨尿干化学白细胞酯酶(leukocyte esterase,LEU)联合亚硝酸盐(nitrite,NIT)快速诊断尿路感染的可靠性。方法非随机选取2019年6月—2024年2月云南省精神病医院收治的96例疑似尿路感染患者作为研究对象,收集尿常规干化学检测结果,以细菌培养结果为金标准,统计LEU、NIT单项及联合检测的诊断效能。结果在尿路细菌感染诊断效能评价中,LEU&NIT并联时灵敏度和阴性预测值分别为90.00%(36/40)和87.88%(29/33),高于LEU单项检测的67.50%(27/40)、66.67%(26/39),及NIT单项检测的45.00%(18/40)、69.44%(50/72),差异有统计学意义(P均<0.05)。LEU&NIT串联时特异度、阳性预测值高于LEU单项检测,差异有统计学意义(P均<0.05)。结论尿常规中白细胞酯酶联合亚硝酸盐检测可避免两种方法单一检测的缺点,提高尿路感染诊断的准确度。Objective To investigate the reliability of urinary dry chemical leukocyte esterase(LEU)combined with nitrite(NIT)in rapid diagnosis of urinary tract infection.Methods A total of ninety-six patients with suspected urinary tract infection admitted to Yunnan Psychiatric Hospital from June 2019 to February 2024 were non-randomly selected as the study objects.The results of routine dry chemical urine test were collected,and the diagnostic efficacy of single and combined LEU and NIT tests were analyzed using bacterial culture results as the gold standard.Results The sensitivity and negative predictive value of LEU&NIT parallel detection were 90.00%(36/40)and 87.88%(29/33),respectively,higher than 67.50%(27/40)and 66.67%(26/39)of LEU single detection,and 45.00%(18/40)and 69.44%(50/72)of NIT single detection,the difference were statistically significant(all P<0.05).The specificity and positive predictive value of LEU&NIT tandem were higher than those of LEU single detection,and the differences were statistically significant(all P<0.05).Conclusion The detection of leucocyte esterase combined with nitrite in urine routine can avoid the shortcomings of the two methods and improve the accuracy of the diagnosis of urinary tract infection.
