摘 要:目的探究经髌上入路闭合复位胫骨髓内钉固定复杂胫骨骨折的疗效。方法非随机选取2022年10月—2023年10月赫章县人民医院收治的60例复杂胫骨骨折患者为研究对象,按治疗方法分为两组,各30例。两组患者均予以闭合复位胫骨髓内钉固定治疗,其中参比组采取经髌下入路,观察组采取经髌上入路。对比两组手术情况、膝关节活动度、膝关节功能、日常生活活动能力与并发症发生情况。结果观察组手术情况、屈膝最大角度、膝关节功能评分及日常生活活动能力评分均优于参比组,差异有统计学意义(P均<0.05)。观察组并发症总发生率为6.67%(2/30),低于参比组的30.00%(9/30),差异有统计学意义(χ^(2)=5.455,P<0.05)。结论经髌上入路闭合复位胫骨髓内钉固定治疗复杂胫骨骨折,可加速术后恢复,提高患者日常生活活动能力,且安全性更高。Objective To explore the curative effect of closed reduction and tibial intramedullary nail fixation of complex tibial fractures through suprapatellar approach.Methods A total of sixty patients with complex tibial fractures admitted to Hezhang County People's Hospital from October 2022 to October 2023 were non-randomly selected as research objects,and according to treatment methods,they were divided into two groups,thirty cases in each group.Both groups were treated with closed reduction and tibial intramedullary nail fixation.The reference group was treated with subpatellar approach,and the observation group was treated with suprapatellar approach.The operation condition,knee joint activity,knee joint function,activities of daily living and complications were compared between the two groups.Results The operation condition,the maximum angle of knee flexion,knee function score and activity of daily living score of the observation group were better than those of the reference group,and the differences were statistically significant(all P<0.05).The total incidence of complications in the observation group was 6.67%(2/30),lower than 30.00%(9/30)in the reference group,and the difference was statistically significant(χ^(2)=5.455,P<0.05).Conclusion Closed reduction and tibial intramedullary nail fixation through suprapatellar approach for complex tibial fractures can accelerate postoperative recovery,improve daily living activities,and have higher safety.
