肾癌根治术与肾部分切除术在T1b期肾癌患者中的应用研究

摘　　要：目的探讨肾癌根治术(radicalnephrectomy,RN)与肾部分切除术(partialnephrectomy,PN)在T1b期肾癌患者中的应用效果。方法非随机选取2016年8月—2022年7月济南市章丘区人民医院收治的90例T1b期肾癌患者为研究对象,按照治疗方法不同分组。观察组45例患者采用PN治疗,对照组45例患者采用RN治疗。针对两组术中与术后指标、血肌酐(serum creatinine,Scr)水平、术后并发症、1年内复发率与生存率进行比较。结果观察组术中出血量低于对照组,差异有统计学意义(P<0.05)。两组手术时间及术后引流、住院时间比较,差异无统计学意义(P均>0.05)。观察组术后Scr水平低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为6.67%(3/45),低于对照组的24.44%(11/45),差异有统计学意义(χ^(2)=5.414,P<0.05)。两组随访1年后复发率与生存率比较,差异无统计学意义(P均>0.05)。结论与RN相比,PN能够有效保护T1b期肾癌患者的肾功能,减少术中出血量,降低并发症风险。Objective To explore the application effects of radical nephrectomy(RN)and partial nephrectomy(PN)in patients with T1b stage renal cell carcinoma.Methods Ninety patients with T1b stage renal cell carcinoma in Jinan Zhangqiu District People's Hospital from August 2016 to July 2022 were non-randomly selected as study subjects.According to different treatment methods,they were divided into two groups with forty-five in each group.The observation group received PN treatment and the control group received RN treatment.Compare intraoperative and postoperative indicators,Scr levels,postoperative complications,1-year recurrence rate,and survival rate between two groups.Results The intraoperative bleeding in the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).There were no statistical significant difference in operation time and postoperative drainage and hospitalization between two groups(all P>0.05).The postoperative Scr level in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The complication rate in the observation group was 6.67%(3/45),lower than 24.44%(11/45)in the control group,and the difference was statistically significant(χ^(2)=5.414,P<0.05).There were no statistically significant difference in recurrence rate and survival rate after 1-year follow-up between the two groups(both P>0.05).Conclusion Compared with RN,PN can effectively reduce intraoperative bleeding,ensure renal function of patients,reduce the risk of complications.
