乳腺叶状肿瘤的CD10、CD117、p53、Ki67表达及其临床意义分析

摘　　要：目的研究分析不同类型乳腺叶状肿瘤(phyllodes tumor,PT)的免疫组化指标CD10、CD117、p53、Ki67表达情况及临床意义。方法回顾性选取2016年1月—2023年10月江苏大学附属句容医院收治的60例乳腺叶状肿瘤患者的临床资料,根据肿瘤性质分为良性组(39例)、交界性组(13例)、恶性组(8例),采用免疫组化技术检测其CD10、CD117、p53、Ki67阳性表达情况,比较三组阳性表达率。结果良性组、交界性组、恶性组的CD10阳性表达率分别为23.08%(9/39)、61.54%(8/13)、87.50%(7/8),三组比较,差异有统计学意义(χ^(2)=14.688,P=0.001)。CD117在良性组、交界性组、恶性组的阳性表达率分别为2.56%(1/39)、7.69%(1/13)、0,三组比较,差异无统计学意义(χ^(2)=1.114,P=0.573)。交界性组、恶性组的p53阳性表达率均高于良性组,差异有统计学意义(P均<0.05)。交界性组、恶性组的Ki67阳性表达率均高于良性组,恶性组的Ki67阳性表达率高于交界性组,差异有统计学意义(P均<0.05)。结论在不同类型乳腺叶状肿瘤患者的乳腺病灶组织中,CD10、P53、Ki67主要在交界性肿瘤、恶性肿瘤中表达,在良性肿瘤中表达较少,而CD117在不同性质肿瘤中表达均少见。Objective To study and analyze the expression and clinical significance of immunohistochemical markers CD10,CD117,p53 and Ki67 in different types of phyllodes tumor(PT).Methods The clinical data of sixty patients with breast phyllodes tumor admitted to Jurong Hospital Affiliated to Jiangsu University from January 2016 to October 2023 were retrospectively selected.According to the nature of the tumor,they were divided into benign group(thirty-nine cases),borderline group(thirteen cases)and malignant group(eight cases).Immunohistochemical technique was used to detect the positive expression of CD10,CD117,p53 and Ki67,and the positive expression rates of the three groups were compared.Results The positive expression rates of CD10 in benign group,borderline group and malignant group were 23.08%(9/39),61.54%(8/13)and 87.50%(7/8),and the differences were statistically significant(χ^(2)=14.688,P=0.001).The positive expression rates of CD117 in benign group,borderline group and malignant group were 2.56%(1/39),7.69%(1/13)and 0,there were no statistically significants differences(χ^(2)=1.114,P=0.573).The positive expression rate of p53 in the borderline group and the malignant group were higher than the benign group,and the differences were statistically significant(all P<0.05).The positive expression rate of Ki67 in borderline group and malignant group were higher than the benign group,and the positive expression rate of Ki67 in malignant group was higher than that in borderline group,and the differences were statistically significant(all P<0.05).Conclusion In breast lesions of patients with different types of breast phyllodes tumors,CD10,p53 and Ki67 were mainly expressed in borderline tumors and malignant tumors,and less expressed in benign tumors,while CD117 was rarely expressed in different types of tumors.
