摘　　要：目的分析成人乙肝疫苗接种中不同剂量国产重组酵母乙肝疫苗作用差异。方法回顾性选取2021年2月—2022年2月临沂市疾病预防控制中心进行疫苗接种的120名接种者的接种资料,根据疫苗接种的不同剂量进行分组,每组60名。观察组使用高剂量重组乙型肝炎疫苗(10μg),对照组使用低剂量重组乙型肝炎疫苗(5μg)。比较两组接种者表面抗体平均几何滴度、乙肝表面抗体(hepatitis B surface antibody,HbsAb)的阳性率、不良反应发生率。结果观察组第1、3、6个月的表面抗体平均几何滴度均高于对照组,差异有统计学意义(P均<0.05)。观察组的HbsAb阳性率为96.67%(58/60),高于对照组的85.00%(51/60),差异有统计学意义(χ^(2)=4.904,P<0.05)。观察组发生红肿1起,对照组发生红肿1起、恶心1起,两组不良反应发生率相比,差异无统计学意义(P>0.05)。结论在接种国产重组酵母乙肝疫苗时,使用10μg剂量效果显著,抗体阳性率较高,且不会产生较多的不良反应,在成人接种乙肝疫苗时可以适量加大剂量。Objective To analyze the difference in the effect of domestic recombinant yeast vaccine in adult HB vacci⁃nation.Methods The vaccination data of 120 inoculants who were vaccinated in Linyi Center for Disease Control and Prevention from February 2021 to February 2022 were retrospectively selected and grouped according to different doses of vaccination,with 60 in each group.The observation group was treated with high-dose recombinant hepatitis B vaccine(10μg),and the control group was treated with low-dose recombinant hepatitis B vaccine(5μg).The aver⁃age geometric titer of surface antibody,the positive rate of hepatitis B surface antibody(HbsAb)and incidence of ad⁃verse reactions were compared between the two groups.Results The average geometric titers of surface antibodies in the observation group were higher than those in the control group at 1,3 and 6 months,and the differences were statis⁃tically significant(all P<0.05).The positive rate of HbsAb in the observation group was 96.67%(58/60),which was higher than 85.00%(51/60)in the control group,and the difference was statistically significant(χ^(2)=4.904,P<0.05).There was 1 case of redness and swelling in the observation group,1 case of redness and swelling in the control group,and 1 case of nausea.There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion In the inoculation of domestic recombinant yeast hepatitis B vaccine,the effect of 10μg dose is significant,the positive rate of antibody is high,and there will be no more adverse reactions.Therefore,when adults are vaccinated with hepatitis B vaccine,the dose can be increased appropriately.
