纤维蛋白原、血清前白蛋白及中性粒细胞/淋巴细胞比值对细菌性肺炎诊断价值研究

摘　　要：目的分析纤维蛋白原(fibrinogen,FIB)、血清前白蛋白(prealbumin,PA)及中性粒细胞/淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)对细菌性肺炎的诊断价值。方法非随机选取2022年2月—2023年6月北海市人民医院收治的120例疑似细菌性肺炎患者为研究对象。对所有患者实施FIB、PA以及NLR的检测,以痰培养结果为金标准,分析单一指标与3项指标联合检测对细菌性肺炎的诊断价值。结果痰培养诊断阳性患者56例,阴性患者64例。FIB、PA、NLR联合诊断的特异度、敏感度、阴性预测值、阳性预测值分别为96.88%(62/64)、100.00%(56/56)、100.00%(62/62)、96.55%(56/58),均高于单一指标诊断,差异有统计学意义(P均<0.05)。结论在细菌性肺炎诊断中应用FIB、PA、NLR的检测方式具有积极意义,且联合指标的价值更高。Objective To analyze the diagnostic value of fibrinogen(FIB),serum prealbumin(PA)and neutrophil-to-lymphocyte ratio(NLR)for bacterial pneumonia.Methods A total of 120 patients with suspected bacterial pneumonia admitted to Beihai People′s Hospital from February 2022 to June 2023 were selected non-randomly as the study sub⁃jects.The tests of FIB,PA and NLR were performed on all patients,and the diagnostic value of single index and the combined test of the three indexes for bacterial pneumonia was analyzed using sputum culture results as the gold stan⁃dard.Results Sputum culture was diagnosed positively in 56 patients and negatively in 64 patients.The specificity,sensitivity,negative predictive value,and positive predictive value of the combined diagnosis of FIB,PA,and NLR were 96.88%(62/64),100.00%(56/56),100.00%(62/62),and 96.55%(56/58),respectively,which were higher than those of the single-indicator diagnosis,and the differences were statistically significant(all P<0.05).Conclusion The application of FIB,PA,and NLR in the diagnosis of bacterial pneumonia is of positive significance,and the value of the combined index is higher.
