腹腔镜胆囊切除和传统开放手术治疗急性化脓性胆囊炎的临床效果分析

摘　　要：目的研究急性化脓性胆囊炎治疗中应用腹腔镜胆囊切除术、传统开放手术的临床效果。方法非随机选取2022年1—12月青岛市胶州中心医院肝胆外科收治的60例急性化脓性胆囊炎患者为研究对象,按治疗方法不同分为两组,每组30例。常规组采用传统开放手术,观察组采用腹腔镜胆囊切除术,对比两组并发症发生率、手术及术后恢复情况、炎症因子水平、胃肠激素水平。结果观察组并发症发生率3.33%(1/30)低于常规组的26.67%(8/30),差异有统计学意义(χ^(2)=4.706,P<0.05)。观察组手术及术后恢复指标、术后炎症因子指标、胃肠激素水平优于常规组,差异有统计学意义(P均<0.05)。结论运用腹腔镜胆囊切除术治疗急性化脓性胆囊炎患者,安全性良好,有助于患者术后恢复。Objective To study the clinical effect of laparoscopic cholecystectomy and traditional open surgery in the treatment of acute suppurative cholecystitis.Methods Sixty patients with acute suppurative cholecystitis admitted to the Department of Hepatobiliary Surgery of Qingdao Jiaozhou Central Hospital from January to December 2022 were non-randomly selected as the study subjects,with thirty cases in each group.According to different treatment meth⁃ods,they were divided into two groups.The conventional group was treated with traditional open surgery,and the ob⁃servation group was treated with laparoscopic cholecystectomy.The incidence of complications,surgical and postopera⁃tive recovery,inflammatory factor levels,and gastrointestinal hormone levels were compared between the two groups.Results The incidence of complications in the observation group was 3.33%(1/30),lower than 26.67%(8/30)of the conventional group,and the difference was statistically significant(χ^(2)=4.706,P<0.05).The surgery and postoperative recovery index,postoperative inflammatory sub-factor index,and gastrointestinal hormone level of the observation group were better compared with the conventional group,and the differences were statistically significant(all P<0.05).Conclusion Laparoscopic cholecystectomy for patients with acute suppurative cholecystitis is safe and helpful for post⁃operative recovery.
