摘　　要：目的:分析超声造影(CEUS)联合弹性成像(UE)对颈部转移性及非转移性淋巴结的鉴别诊断价值。方法:选取2021年3月~2024年3月于我院就诊的颈部淋巴结疾病患者117例作为研究对象,均行CEUS与UE检查,根据淋巴结是否转移分为转移组与非转移组,比较两组患者CEUS、UE相关参数,同时以病理结果作为金标准,比较CEUS、UE单独检查或联合检查对颈部淋巴结转移或非转移的诊断准确性。结果:共有43例患者颈部淋巴结发生转移,转移组与非转移组增强方式比较差异无统计学意义(P>0.05),但与非转移组比较,转移组不均匀增强、无或低增强占比、蓝色面积比值、弹性应变率显著高(P<0.05),与单独行CEUS或UE检查而言,二者联合检查对颈部淋巴结是否转移的诊断敏感度、特异度及准确度均较高(P<0.05)。结论:CEUS联合UE对颈部转移性或非转移性淋巴结具有较高的鉴别诊断价值。Objective:To analyze the value of contrast-enhanced ultrasound(CEUS)combined with ultrasound elastography(UE)in differential diagnosis of metastatic and non-metastatic cervical lymph nodes.Methods:A total of 117 patients with cervical lymph node lesions who visited the hospital from March 2021 to March 2024 were selected as the research subjects.All of them received CEUS and UE examinations.They were divided into the metastasis group and the non-metastasis group according to whether the lymph nodes were metastatic.CEUS and UE parameters of the two groups were compared.Pathological results were used as the gold standard to compare the diagnostic accuracy of CEUS,UE and their combination for cervical lymph node metastasis.Results:There were 43 patients with cervical lymph node metastasis.There was no statistically significant difference in enhancement mode between the metastasis group and the non-metastasis group(P>0.05).Compared with the non-metastasis group,the proportions of inhomogeneous enhancement,no or low enhancement,blue area ratio,and elastic strain rate were significantly higher in the metastasis group(P<0.05).Compared with CEUS or UE,combined examination with the two could achieve higher sensitivity,specificity and accuracy in diagnosing cervical lymph node metastasis(P<0.05).Conclusion:CEUS combined with UE has high value in differential diagnosis of metastatic and non-metastatic cervical lymph nodes.
