摘  要:目的:探讨胃癌合并糖尿病术后辅助化疗患者使用多学科协作管理模式联合回授式健康教育的应用效果。方法:选取2022年1月至2024年1月我院收治100例胃癌合并糖尿病手术治疗后辅助化学治疗患者作为研究对象,随机分为对照组和观察组,各50例。对照组患者使用多学科管理模式进行干预,观察组患者在对照组的基础上联合使用回授式健康教育,两组患者均干预12 w。比较两组患者干预前后血糖控制情况[糖化血红蛋白(Hemoglobin Type A1C,HbA1c)、餐后2 h血糖(2 h Postprandial Plasma Glucose,2hPPG)、空腹血糖(Fasting Blood Glucose,FBG)]、营养管理情况[血清总白蛋白(Total Protein,TP)、前白蛋白(Prealbumin,PA)、白蛋白(Albumin,ALB)]自我管理情况[慢性病健康素养量表(Health Literacy Management Scale,HeLMS)、糖尿病自我行为管理量表(Summary of Diabetes Self Care Activities,SDSCA)]。结果:两组患者干预前血糖及营养水平,自我管理水平无差别,干预后两组患者均有改善。观察组患者干预后TP、PA、ALB水平及SDSCA、HeLMS评分较高,HbA1c、2hPPG、FBG水平较低(P<0.05)。结论:胃癌合并糖尿病术后辅助化疗患者使用多学科协作管理模式联合回授式健康教育效果良好,可改善患者健康素养,提升患者自我行为管理水平,同时使患者血糖得到有效控制,改善患者营养状态。Objective:To explore the application effect of multidisciplinary collaborative management model combined with feedback health education in patients with gastric cancer complicated with postoperative adjuvant chemotherapy for diabetes.Methods:From January 2022 to January 2024,100 patients with gastric cancer complicated with diabetes treated by adjuvant chemotherapy in our hospital were selected as the research object and randomly divided into control group and observation group,with 50 cases in each group.Patients in the control group were intervened by multidisciplinary management mode,while patients in the observation group were combined with feedback health education on the basis of the control group.Both groups were intervened for 12 weeks.The blood sugar control of the two groups was compared before and after the intervention[Hemoglobin Type A1C(HbA1c),2h Postprandial Plasma Glucose(2hPPG),Fasting Blood Glucose(FBG)],nutrition management[total Albumin(TP),Prealbumin(PA),albumin(ALB)]self-management[Health Literacy Management Scale for Chronic Diseases(Helms),Diabetes Self-care Activities;SDSCA)].Results:There was no difference in the level of blood glucose,nutrition and self-management between the two groups before intervention,and both groups improved after intervention.After intervention,the levels of TP,PA,ALB,SDSCA and HeLMS were higher in observation group,while the levels of HbA1c,2hPPG and FBG were lower(P<0.05).Conclusion:For patients with gastric cancer complicated with postoperative adjuvant chemotherapy for diabetes mellitus,the combination of multidisciplinary collaborative management mode and feedback health education has good effects,which can improve the health literacy of patients,enhance the level of self-behavior management of patients,effectively control the blood sugar of patients,and improve the nutritional status of patients.
