摘 要:目的:探究可溶性白细胞分化抗原14亚型(Soluble leukocyte cluster of differentiation 14 subtype,sCD14-ST)联合降钙素原(Procalcitonin,PCT)、超敏C反应蛋白(High-sensitivity C-reactive protein,hs-CRP)对细菌性呼吸道感染患儿的诊断价值。方法:选取2023年1月至2024年12月于本院就诊的60例细菌性呼吸道感染患儿作为研究对象的研究组,另选取同期于本院接受体检的60例健康儿童作为研究对象的对照组。比较两组的血清sCD14-ST、PCT和hs-CRP水平及研究组治疗前后的上述指标水平,分析其中与儿童出现细菌性呼吸道感染密切相关的因素,并利用受试者工作特征(Receiver operating characteristic,ROC)曲线评估上述血清学指标对细菌性呼吸道感染患儿的诊断价值。结果:与对照组比较,研究组血清sCD14-ST、PCT和hs-CRP水平均明显较高(P<0.05)。研究组治疗后上述指标的水平与治疗前相比均明显降低(P<0.05)。多元Logistic回归分析表明,血清sCD14-ST、PCT和hs-CRP水平异常升高均为儿童出现细菌性呼吸道感染的危险因素(P<0.05)。经ROC分析发现,血清sCD14-ST、PCT和hs-CRP联合检测儿童出现细菌性呼吸道感染的曲线下面积(Area under roc curve,AUC)明显高于单一指标(P<0.05)。结论:血清sCD14-ST、PCT和hs-CRP水平的联合测定在诊断儿童细菌性呼吸道感染方面具有较高的应用价值。Objective:to explore diagnostic value of soluble leukocyte cluster of differentiation 14 subtype(sCD14-ST)combined with procalcitonin(PCT)and high-sensitivity C-reactive protein(hs-CRP)in children with bacterial respiratory tract infection.Methods:Sixty children with bacterial respiratory infections who received treatment at our hospital from January 2023 to December 2024 were selected as the research subjects and included in study group.Additionally,60 healthy children who underwent physical examinations at our hospital during the same period were selected as control group.The levels of serum sCD14-ST,PCT and hs-CRP were compared between the two groups.The influencing factors of bacterial respiratory tract infection were explored by multivariate Logistic regression analysis,and their diagnostic value in children with bacterial respiratory tract infection was evaluated by ROC curves.Results:Compared with control group,levels of serum sCD14-ST,PCT and hs-CRP were higher in study group(P<0.05).Multivariate Logistic regression analysis showed that abnormally increased levels of serum sCD14-ST,PCT and hs-CRP were risk factors of bacterial respiratory infection(P<0.05).ROC curves analysis showed that AUC of sCD14-ST combined with PCT and hs-CRP in the diagnosis of bacterial respiratory tract infection was 0.879,greater than that of single index(P<0.05).Conclusion:The combined detection of serum sCD14-ST,PCT and hs-CRP has high diagnostic value in children with bacterial respiratory tract infection.
