丙酚替诺福韦治疗慢性乙型肝炎低病毒血症患者的病毒学抑制率及ALT复常率研究

摘　　要：目的:研究丙酚替诺福韦(Tenofovir alafenamide fumarate,TAF)治疗慢性乙型肝炎(Chronic hepatitis B,CHB)低病毒血症(Low-level viremia,LLV)患者对其病毒学抑制率及丙氨酸转氨酶(Alanine aminotransferase,ALT)复常率的影响。方法:选取2021年1月至2024年1月在我院就诊的150例CHB-LLV患者纳入研究,按倾向评分分为使用丙酚替诺福韦治疗的TAF组(n=75)以及使用恩替卡韦(entecavir,ETV)治疗的ETV组(n=75)。治疗24 w后比较两组病毒学抑制率、肝功能情况、ALT复常率以及不良反应发生情况。结果:TAF组治疗后乙型肝炎病毒(Hepatitis B Virus,HBV)-DNA、ALT、总胆红素(Total bilirubin,TBIL)、天门冬氨酸氨基转移酶(Aspartate aminotransferase,AST)水平均明显小于ETV组(P<0.05);HBV-DNA转阴率、ALT复常率均明显高于ETV组(P<0.05);两组不良反应发生率相当。结论:TAF治疗CHB-LLV效果明显,能明显提高病毒学抑制率与ALT复常率,改善患者肝功能,安全可靠。Objective:To investigate the effect of tenofovir alafenamide(TAF)on virological suppression rate and alanine aminotransferase(ALT)normalization rate in patients with chronic hepatitis B(CHB)-low-level viremia(LLV).Methods:A total of 150 patients with CHB-LLV in our hospital from January 2021 to January 2024 were enrolled in the study.They were assigned to TAF group(75 patients treated with TAF)and ETV group(75 patients treated with entecavir)using the propensity score matching.The virological suppression rate,liver function,ALT normalization rate,and the incidence of adverse reactions were compared between the two groups after 24 weeks of treatment.Results:After treatment,the levels of hepatitis B virus(HBV-DNA),ALT,total bilirubin(TBIL),and aspartate aminotransferase(AST)in TAF group were significantly lower than those in ETV group(P<0.05).The negative conversion rates of HBV-DNA,and ALT normalization rate were significantly higher than those in ETV group(P<0.05).The incidence rates of adverse reactions in the two groups were comparable.Conclusion:TAF is markedly effective in the treatment of CHB-LLV,which can significantly increase virological suppression rate and ALT normalization rate,and improve patients’liver function.It is safe and reliable.
