切开复位内固定术对踝关节骨折患者关节功能恢复效果的影响

摘　　要：目的:分析踝关节骨折应用切开复位内固定术治疗的效果。方法:选取2023年1月至12月期间在本院接受治疗的踝关节骨折病例80例,随机分为对照组和观察组(n=40),分别予以常规治疗和切开复位内固定治疗。治疗3 m后,采用采用美国足踝外科协会踝-后足评分系统(American orthopedic foot and ankle society,AOFAS)评分和Baird-Jackson踝关节评分系统评定患者踝关节功能,应用量角器测量患者的踝关节活动度,采用生活质量测定简表(World Health organization quality of life brief questionnaire,WHOQOL-BREF)评定生活质量,并比较两组骨折愈合时间。结果:与治疗前相比,两组治疗后AOFAS、Baird-Jackson评分均显著升高,且观察组AOFAS、Baird-Jackson评分均显著高于对照组(P<0.05)。与治疗前相比,两组治疗后背伸、跖屈、内翻和外翻活动度数值均显著增加,且观察组显著高于对照组(P<0.05)。观察组的骨折愈合时间显著短于对照组(P<0.05)。与治疗前相比,两组治疗后生活质量评分均升高,且观察组显著高于对照组(P<0.05)。结论:实施切开复位内固定术,能够缓解患者伤情,改善患者踝关节功能,促进骨折愈合,改善生活质量。Objective:To analyze the effect of open reduction and internal fixation for ankle fractures.Methods:A total of 80 cases of ankle fractures treated in our hospital from January to December 2023 were randomly divided into a control group and an observation group(n=40),and were treated with conventional therapy and open reduction and internal fixation,respectively.After 3 months of treatment,the American Orthopedic Foot and Ankle Society(AOFAS)ankle-hindfoot scoring system and the Baird-Jackson ankle scoring system were used to evaluate the patient's ankle function.The patient's ankle range of motion was measured using a protractor,and the World Health Organization Quality of Life Brief Questionnaire(WHOQOL-BREF)was used to assess quality of life.The fracture healing time was compared between the two groups.Results:Compared with before treatment,the AOFAS and Baird-Jackson scores in both groups increased significantly after treatment,and the AOFAS and Baird-Jackson scores in the observation group were significantly higher than those in the control group(P<0.05).Compared with before treatment,the values of back extension,plantar flexion,varus and valgus activity in both groups after treatment increased significantly,and the observation group was significantly higher than the control group(P<0.05).The fracture healing time in the observation group was significantly shorter than that in the control group(P<0.05).Compared with before treatment,the quality of life scores of both groups increased after treatment,and the observation group was significantly higher than the control group(P<0.05).Conclusion:The implementation of open reduction and internal fixation can alleviate the patient's injury,improve the patient's ankle joint function,promote fracture healing,and improve quality of life.
