摘 要:目的:探讨基于Roy适应模式辅助干预对重症肺炎患者症状、适应能力及炎症水平的影响。方法:采用随机数字表法将2021年3月至2023年9月于我科治疗的112例重症肺炎患者分为对照组(56例)与ROY组(56例),对照组给予个体优化干预,ROY组在对照组的基础上开展Roy适应模式辅助干预,干预周期2 w,对比两组的症状、简易罗伊应对适应量表(Coping-adaptation processing scale,CAPS-15)评分、炎症水平及不良事件。结果:干预2 w后,ROY组的咳嗽消退时间、发热消退时间短于对照组(P<0.05);ROY组的急性生理与慢性健康评分II(Acute Physiology and Chronic Health Evaluation II,APACHE II)低于对照组(P<0.05);ROY组的CAPS-15评分高于对照组(P<0.05);ROY组的白细胞计数、白介素-8(Interlwukin-8,IL-8)、白介素-6(Interlwukin-6,IL-6)的水平低于对照组(P<0.05);ROY组的不良事件发生率低于对照组(P<0.05)。结论:Roy适应模式辅助干预能够有效改善重症肺炎患者的症状、炎症水平,提高患者的适应能力,减少不良事件的发生。Objective:Objective:To explore the effect of Roy-based adaptation model on symptoms,adaptability and inflammatory level of patients with severe pneumonia.Methods:112 patients with severe pneumonia treated in our department from March 2021 to September 2023 were randomly divided into control group(56 cases)and ROY group(56 cases).The control group received individual optimization intervention,and ROY group received Roy adaptation mode intervention based on the control group.The intervention cycle was 2 weeks.Symptoms(cough resolution time,fever resolution time,APACHE II score),Simple Roy Coping Adaptation Scale(CAPS-15)score,inflammation level and adverse events were compared between the two groups.Results:After 2 weeks of intervention,the time of cough resolution and fever resolution in ROY group was shorter than that in control group(P<0.05).The acute physiology and chronic health score II of ROY group was lower than that of control group(P<0.05).The CAPS-15 score of ROY group was higher than that of control group(P<0.05).The levels of leukocyte count,interleukin-8(IL-8)and interleukin-6(IL-6)in ROY group were lower than those in control group(P<0.05).The incidence of adverse events in ROY group was lower than that in control group(P<0.05).Conclusion:Roy adaptation model can effectively improve the symptoms and inflammation level of patients with severe pneumonia,improve their adaptability and reduce the occurrence of adverse events.
