摘  要:目的:研究无创机械通气(Non-invasive positive pressure ventilation,NIPPV)治疗慢性阻塞性肺疾病急性发作(Acute exacerbation of chronic obstructive pulmonary disease,AECOPD)效果评价及对血气指标的影响。方法:选取2022年6月至2024年6月期间本院收治的70例AECOPD患者作为研究对象,通过随机数字表法将患者分为无创组与吸氧组,各35例。无创组以NIPPV治疗,吸氧组以鼻导管吸氧治疗作为对照。比较无创组与吸氧组采用NIPPV治疗前1 d与治疗14 d时的胸肺顺应性[总顺应性(Total compliance,Ct)、肺顺应性(Compliance of lung,Cl)和胸廓顺应性(Thoracic compliance,CTh)]、血气[动脉血氧分压(Partial pressure of oxygen in arterial blood,PaO_(2))和动脉血二氧化碳分压(Partial pressure of carbon dioxide in arterial blood,PaCO_(2))]水平、炎症水平[白细胞介素-6(Interleukin-6,IL-6)和降钙素原(Procalcitonin,PCT)]以及不良反应。结果:治疗后,无创组Ct、Cl、CTh、PaO_(2)水平均明显高于吸氧组(P<0.05);无创组PaCO_(2)水平、IL-6和PCT水平明显低于吸氧组(P<0.05)。两组不良反应无明显差异(P>0.05)。结论:采用NIPPV治疗AECOPD可以更好的促进患者血气指标恢复,改善患者的肺通气功能,有效控制炎症反应,未增加不良反应风险。Objective:To study curative effect of Non-invasive positive pressure ventilation(NIPPV)and its influences on blood gas indexes in Acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:A total of 70 patients with AECOPD admitted to the hospital were enrolled as the research objects between June 2022 and June 2024.According to random number table method,they were divided into non-invasion group(NIPPV)and oxygen inhalation group(oxygen inhalation through nasal catheter),35 cases in each group.The thoracic and lung compliance[Total compliance(Ct),Compliance of lung(Cl),Thoracic compliance(CTh)],blood gas indexes[Partial pressure of oxygen in arterial blood(PaO_(2)),partial Pressure of carbon dioxide in arterial blood(PaCO_(2))],inflammatory level[Interleukin-6(IL-6),Procalcitonin(PCT)]and adverse reactions were compared between the two groups before and after 14 days of treatment.Results:After treatment,the levels of Ct,Cl,CTh,and PaO_(2)in the non-invasive group were significantly higher than those in the oxygen therapy group(P<0.05).The levels of PaCO_(2),IL-6,and PCT in the non-invasive group were significantly lower than those in the oxygen therapy group(P<0.05).There were no significant differences in adverse reactions between the two groups(P>0.05).Conclusion:NIPPV can better promote recovery of blood gas indexes,improve lung ventilation function and effectively control inflammatory response in AECOPD patients,with low risk of adverse reactions.
