溶血三项试验、胆红素水平在新生儿ABO溶血病中的检测意义

摘　　要：目的:分析溶血三项试验与胆红素水平在新生儿ABO溶血病(ABO-HDN)中的检测意义。方法:选取2021年6月~2024年6月于我院接受治疗的疑似ABO-HDN的新生儿60例,根据溶血三项试验(抗体放散试验、直接抗球蛋白试验、游离抗体试验)检测结果分为ABO-HDN组和非ABO-HDN组,比较两组患儿胆红素[总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)]水平,并采用多因素logistic回归分析、受试者工作特征(ROC)曲线分析TBIL、DBIL、IBIL与ABO-HDN的相关性及其预测价值。结果:溶血三项试验检测中抗体放散试验阳性47例,直接抗球蛋白试验阳性20例,游离抗体试验阳性40例,共确诊47例ABO-HDN患儿,检测阳性率分别为100%、42.55%、85.11%,与非ABO-HDN组比较,ABO-HDN组TBIL、DBIL、IBIL水平显著高(P<0.05),多因素logistic回归分析表明TBIL、DBIL、IBIL是ABO-HDN的独立影响因素(P<0.05),ROC曲线分析显示TBIL、DBIL、IBIL预测ABO-HDN的曲线下面积分别为0.761、0.796、0.791。结论:溶血三项试验及胆红素水平在新生儿ABO-HDN检测中具有较高的诊断价值,有助于临床及时发现ABO-HDN并采取相应治疗措施。Objective:To analyze the significance of three hemolysis tests and bilirubin level in detecting ABO hemolytic disease of the newborn(ABO-HDN).Methods:Sixty newborns with suspected ABO-HDN admitted to the hospital from June 2021 to June 2024 were selected.They were divided into ABO-HDN group and non-ABO-HDN group based on the results of three hemolysis tests(release test,direct antiglobulin test and free antibody test).Bilirubin[total bilirubin(TBIL),direct bilirubin(DBIL)and indirect bilirubin(IBIL)]levels in the two groups were compared.Multivariate logistic regression analysis was used to investigate the correlation between TBIL,DBIL,IBIL and ABO-HDN.The receiver operating characteristic(ROC)curves were used to evaluate the predictive value of above indexes.Results:47 cases were positive for release test,20 cases were positive for direct antiglobulin test,and 40 cases were positive for free antibody test.A total of 47 cases of ABO-HDN were diagnosed.The positive detection rates were 100%,42.55%and 85.11%.Compared with the non-ABO-HDN group,the levels of TBIL,DBIL and IBIL were significantly higher in the ABO-HDN group(P<0.05).Multivariate logistic regression analysis showed that TBIL,DBIL,and IBIL were independent influencing factors of ABO-HDN(P<0.05).ROC curve analysis showed that the areas under the curve of TBIL,DBIL,and IBIL for predicting ABO-HDN were 0.761,0.796,and 0.791.Conclusion:Three hemolysis tests and bilirubin level are helpful in the detection of ABO-HDN.They can help to detect ABO-HDN in clinical practice and take corresponding treatment measures.
