摘 要:目的:探讨血清维生素D、妊娠相关血浆蛋白A(PAPP-A)、胎盘生长因子(PLGF)预测胎儿生长受限的临床价值。方法:回顾性收集82例于2022年11月至2024年9月期间在本院就诊并分娩的孕妇的临床资料,根据胎儿生长发育情况分为生长受限组(39例)和正常组(43例)。收集孕妇的一般资料和实验室检测指标。对比分析两组孕妇血清维生素D、PAPP-A、PLGF的差异及其对胎儿生长受限的预测价值。结果:生长受限组孕妇的血清维生素D、PAPP-A、PLGF水平均低于正常组(P<0.05)。ROC曲线分析显示,血清维生素D、PAPP-A、PLGF对胎儿生长受限均有预测价值,其中PAPP-A的曲线下面积(AUC)最高,显著高于血清维生素D(Z=1.968、P=0.049)和PLGF(Z=3.499、P=0.000),且血清维生素D的AUC显著高于PLGF(Z=2.527、P=0.011),以3.11 ng•mL^(-1)为截断值,PAPP-A预测胎儿生长受限的敏感度、特异度分别为97.44%、97.67%。结论:血清维生素D、PAPP-A、PLGF在预测胎儿生长受限中具有较高的临床价值,可为临床早期预测胎儿生长受限的潜在指标。Objective:To investigate the clinical value of serum vitamin D,pregnancy-associated plasma protein A(PAPP-A)and placental growth factor(PLGF)in predicting fetal growth restriction.Methods:Clinical data of 82 pregnant women who gave birth in our hospital from November 2022 to September 2024 were retrospectively collected and divided into growth restriction group(39 cases)and normal group(43 cases)according to fetal growth and development.General data and laboratory test indicators of pregnant women were collected.The differences of serum vitamin D,PAPP-A,PLGF and their predictive value for fetal growth restriction were compared between the two groups.Results:The levels of serum vitamin D,PAPP-A and PLGF in growth restriction group were lower than those in normal group(P<0.05).ROC curve analysis showed that serum vitamin D,PAPP-A and PLGF all had predictive value for fetal growth restriction,and the area under the curve(AUC)of PAPP-A was the highest,significantly higher than that of serum vitamin D(Z=1.968,P=0.049)and PLGF(Z=3.499,P=0.000).The AUC of serum vitamin D was significantly higher than that of PLGF(Z=2.527,P=0.011),and the sensitivity and specificity of PAPP-A in predicting fetal growth restriction were 97.44%and 97.67%,respectively,using 3.11 ng/mL as the cut-off value.Conclusion:Serum vitamin D,PAPP-A and PLGF have high clinical value in predicting fetal growth restriction,and can be potential indicators for early clinical prediction of fetal growth restriction.
