火针加中药外敷联合治疗带状疱疹后遗神经痛的疗效观察

摘　　要：目的:探究火针加中药外敷联合治疗带状疱疹后遗神经痛(PHN)的临床疗效。方法:从信宜市慢性病防治站中医科收集2023年4月—2024年4月期间接受治疗的PHN患者108例,依据电脑数字表法分为治疗组(n=54)与参照组(n=54)。参照组给予常规西药治疗;治疗组基于参照组疗法给予火针加中药外敷联合治疗。持续治疗7天,比较两组临床疗效、治疗前后疼痛视觉模拟评分法(VAS)评分及匹兹堡睡眠质量指数(PSQI)评分、不良反应与复发情况。结果:相比治疗前,两组疼痛VAS评分、睡眠PSQI评分治疗后均下降,且治疗组降低幅度高于参照组(P<0.05)。相比参照组有效率(59.26%),治疗组临床疗效有效率(96.30%)更高(P<0.05)。相比参照组,治疗组的不良反应发生率、复发率更低(P<0.05)。结论:带状疱疹后遗神经痛采用火针加中药外敷联合治疗的疗效显著,利于缓解疼痛、改善睡眠质量,避免发生不良反应及复发,值得推广。Objective:To explore the clinical effect of acupuncture plus Chinese medicine on postherpetic neuralgia(PHN).Methods:108 PHN patients who received treatment from April 2023 to April 2024 were collected from the Department of Traditional Chinese Medicine of Xinyi Chronic Disease Prevention and Control Station,and were divided into the treatment group(n=54)and the reference group(n=54)according to the computer number table method.The reference group was treated with conventional western medicine.The treatment group was treated with fire needling combined with external application of traditional Chinese medicine based on the treatment of the reference group.The treatment lasted for 7 days.The clinical efficacy,pain visual analogue scale(VAS)score and Pittsburgh sleep quality index(PSQI)score before and after treatment,adverse reactions and recurrence were compared between the two groups.Results:Compared with before treatment,the pain VAS score and sleep PSQI score of both groups decreased after treatment,and the reduction range of the treatment group was higher than that of the reference group(P<0.05).Compared with the reference group(59.26%),the clinical effective rate of the treatment group(96.30%)was higher(P<0.05).Compared with the reference group,the incidence of adverse reactions and recurrence were lower in the treatment group(P<0.05).Conclusion:The combined treatment of postherpetic neuralgia with fire needle and Chinese medicine is effective,which is beneficial to relieving pain,improving sleep quality,avoiding adverse reactions and recurrence,and is worth promoting.
