血清LncRNA LOXL1-AS1、LncRNA FAF水平与冠心病患者冠状动脉狭窄程度相关性及其对预后的影响

摘  要:目的探讨血清长链非编码RNA(LncRNA)赖氨酸氧化酶样1反义RNA 1(LOXL1-AS1)、LncRNA成纤维细胞生长因子9相关因子(FAF)水平与冠心病患者冠状动脉狭窄(CAS)程度相关性及其对预后的影响。方法选取2021年3月至2023年9月南方医科大学附属中山市博爱医院心血管内科收治的173例冠心病患者(冠心病组)和60例体检健康者(对照组),冠心病患者根据Gensini评分分为轻度CAS组(≤20分,71例)、中度CAS组(21~60分,62例)、重度CAS组(≥61分,40例),根据3个月预后分为不良预后组(35例)和良好预后组(138例),采用实时荧光定量PCR检测血清LncRNA LOXL1-AS1、LncRNA FAF水平。通过Spearman法分析血清LncRNA LOXL1-AS1、LncRNA FAF水平与冠心病患者Gensini评分的相关性,多因素Logistic回归和ROC曲线分析冠心病患者不良预后的因素及血清LncRNA LOXL1-AS1、LncRNA FAF水平对其的预测价值。结果与对照组比较,冠心病组血清LncRNA LOXL1-AS1水平升高,LncRNA FAF水平降低(P<0.05)。轻度CAS组、中度CAS组、重度CAS组血清LncRNA LOXL1-AS1水平依次升高,LncRNA FAF水平依次降低(P<0.05)。血清LncRNA LOXL1-AS1水平与冠心病患者Gensini评分呈正相关,LncRNA FAF水平与冠心病患者Gensini评分呈负相关(P<0.05)。冠心病患者不良预后的独立危险因素为急性心肌梗死、多支病变、Gensini评分增加、LncRNA LOXL1-AS1升高,独立保护因素为LncRNA FAF升高(P<0.05)。血清LncRNA LOXL1-AS1、LncRNA FAF水平联合预测冠心病患者不良预后的曲线下面积为0.870,大于血清LncRNA LOXL1-AS1、LncRNA FAF水平单独预测的0.784、0.792(P<0.05)。结论冠心病患者血清LncRNA LOXL1-AS1水平升高,LncRNA FAF水平降低,与CAS程度加重和不良预后密切相关,血清LncRNA LOXL1-AS1、LncRNA FAF水平联合对冠心病患者预后的预测价值较高。Objective To investigate correlation between serum long non-coding RNA(LncRNA)lysyl oxidase 1 antisense RNA 1(LOXL1-AS1)and LncRNA fibroblast growth factor 9-related factor(FAF)and the degree of coronary artery stenosis(CAS)in patients with coronary artery disease(CAD)and their prognostic implications.Methods From March 2021 to September 2023,173 patients with coronary artery disease(coronary artery disease group)and 60 physically examined healthy persons(control group)who were admitted to the Department of Cardiovascular Medicine of Zhongshan Boai Hospital Offiliated to Southern Medical University were selected,and patients with coronary artery disease were divided into a mild CAS group(≤20 points,n=71),a moderate CAS group(21 to 60 points,n=62),and a severe CAS group(≥61 points,n=40).According to the 3-month prognosis,they were divided into a poor prognosis group(n=35)and a good prognosis group(n=138).Serum LncRNA LOXL1-AS1 and LncRNA FAF were detected by real-time fluorescence quantitative PCR.The correlation between serum LncRNA LOXL1-AS1 and LncRNA FAF and Gensini score in patients with coronary artery disease was analyzed by Spearman s method,and factors of poor prognosis in patients with coronary artery disease and serum LncRNA LOXL1-AS1 and LncRNA FAF were analyzed by multifactorial Logistic regression and ROC curve for their predictive value.Results Compared with control group,serum LncRNA LOXL1-AS1 were increased and LncRNA FAF were decreased in the coronary artery disease group(P<0.05).Serum LncRNA LOXL1-AS1 were sequentially increased and LncRNA FAF were sequentially decreased in the mild CAS group,moderate CAS group,and severe CAS group(P<0.05).Serum LncRNA LOXL1-AS1 were positively correlated with Gensini scores in patients with coronary artery disease,and LncRNA FAF were negatively correlated with Gensini scores in patients with coronary artery disease(P<0.05).Independent risk factors for poor prognosis in patients with coronary artery disease were acute myocardial infarction,multibranch lesion
