双极射频技术结合体外冲击波治疗跖筋膜炎的临床研究

摘　　要：目的通过观察双极射频技术结合体外冲击波治疗跖筋膜炎的临床疗效,探讨双极射频技术结合体外冲击波治疗跖筋膜炎的可行性、安全性及有效性。方法选取符合适应证,于2020年1月至2022年7月在我院就诊患者,收集资料,随机数字表法分为治疗组40例和对照组40例。给予对照组患者体外冲击波治疗,治疗组在对照组基础上联合双极射频术治疗,1周为1个疗程,共计观察4个疗程。比较两组患者治疗前后的临床疗效,疼痛视觉模拟(VAS)评分、足踝功能评分(FAAM)、压痛评分、美国足踝外科协会踝-后足评分(AOFAS)及血清C反应蛋白(CRP)、血清淀粉样蛋白(SAA)和降钙素原(PCT)水平。结果治疗组总有效率明显高于对照组(χ^(2)=5.165,P=0.023)。治疗后,两组VAS评分及压痛评分均低于治疗前,而FAAM及AOFAS评分高于治疗前(P<0.05)。治疗后,血清CRP、SAA、PCT水平均低于治疗前(P<0.05),且治疗组血清CRP、SAA、PCT水平改善明显(P<0.05)。结论双极射频技术结合体外冲击波治疗跖筋膜炎的临床效果较为理想,安全性高。Objective To observe clinical effects of bipolar radiofrequency technology combined with extracorporeal shock wave to treat plantar fasciitis,and to explore feasibility,safety,and effectiveness of the combined therapy for plantar fasciitis.Methods From January 2020 to July 2022,patients who meet indications were collected and treated at our hospital.Data was collected and randomly separated into a therapy group of 40 cases and a reference group of 40 cases by random number table method.Patients in reference group were treated with extracorporeal shock wave therapy,while therapy group received bipolar radiofrequency surgery in addition to the reference group.One week was one course of treatment,and a total of four courses were observed.Clinical efficacy,visual analogue scale(VAS),foot and ankle ability measure(FAAM),tenderness score,American Orthopaedic Foot and Ankle Society(AOFAS)score,serum C-reactive protein(CRP),serum amyloid A(SAA),and procalcitonin(PCT)before and after treatment were compared.Results The total effective rate of therapy group was greatly higher than that of reference group(χ^(2)=5.165,P=0.023).After treatment,VAS score and tenderness score of both groups were lower than before treatment,while FAAM and AOFAS score were higher than before treatment(P<0.05).After treatment,serum CRP,SAA,and PCT were lower than before treatment(P<0.05),and therapy group showed great improvement in serum CRP,SAA,and PCT(P<0.05).Conclusion Clinical effect of bipolar radiofrequency technology combined with extracorporeal shock wave for plantar fasciitis would be good and have high safety.
