摘 要:目的探究尿液镜检与尿液干化学检验联合应用于尿路结石诊断中的价值。方法非随机选取2023年1—11月宿迁市第一人民医院收治的疑似尿路结石的152例患者为研究对象。所有患者均接受尿液镜检、尿液干化学单独及联合检验,以影像学检查作为金标准,比较尿液镜检、尿液干化学检验单独及联合检测的检验效能。结果影像学检查显示,阳性137例、阴性15例;尿液镜检阳性128例、阴性24例;尿液干化学检验阳性133例、阴性19例;联合检测阳性136例、阴性16例。联合检测的准确度为98.03%(149/152),高于尿液干化学检验的86.84%(132/152)和尿液镜检的84.87%(129/152),差异有统计学意义(χ^(2)=16.876,P<0.001),此外联合检测的特异度和灵敏度均高于尿液镜检、尿液干化学检验单独检测,差异有统计学意义(P均<0.05)。结论尿液镜检与尿液干化学检验联合应用于尿路结石诊断中具有较高应用价值。联合检验不仅提高诊断准确度及灵敏度,还降低误诊及漏诊的风险。Objective Exploring the value of the combined application of urine microscopy and urine dry chemical test in the diagnosis of urinary tract stones.Methods A total of 152 patients with suspected urinary calculi admitted to Suqian First People's Hospital from January to November 2023 were non-randomly selected as the study subjects.All patients underwent urine microscopy,urine dry chemistry alone and combined tests.Imaging examination was used as the gold standard to compare the efficacy of the above urine microscopy,urine dry chemistry test alone and combined tests.Results Imaging examination showed that 137 cases were positive and 15 cases were negative.Urine microscopic examination was positive in 128 cases and negative in 24 cases.Urine dry chemistry test was positive in 133 cases and negative in 19 cases.The combined detection was positive in 136 cases and negative in 16 cases.The accuracy of combined detection was 98.03%(149/152),which was higher than 86.84%(132/152)of urine dry chemistry test and 84.87%(129/152)of urine microscopy,and the difference was statistically significant(χ^(2)=16.876,P<0.001).The specificity and sensitivity of combined detection were higher than those of urine microscopy and urine dry chemistry test alone,and the differences were statistically significant(both P<0.05).Conclusion The combination of urine microscopy and urine dry chemistry test has high application value in the diagnosis of urinary calculi.The combined test not only improves the diagnostic accuracy and sensitivity,but also reduces the risk of misdiagnosis and missed diagnosis.
