摘 要:目的探讨血脂、胱抑素C(cystatin C,Cys C)及同型半胱氨酸(homocysteine,Hcy)在冠状动脉粥样硬化性心脏病(coronary heart disease,CHD)诊断中的联合应用价值。方法回顾性选取2022年3月—2024年3月滨州医学院附属医院就诊的81例CHD患者的临床资料作为探究组,并收集同期97名健康体检者的临床资料作为对照组。均进行血脂、Cys C及Hcy的检测。对比两组间各指标的水平差异,并评估上述指标单独及联合检测的诊断效能。结果探究组血脂、Cys C、Hcy水平均更高,差异有统计学意义(P均<0.05)。联合检测的诊断灵敏度97.53%(79/81)、特异度96.91%(94/97)、准确度97.19%(173/178)、阳性预测值96.34%(79/82)、阴性预测值97.92%(94/96),均高于单独检测,差异有统计学意义(χ^(2)=6.767、7.339、8.021、6.823、7.142,P均<0.05)。结论血脂、Cys C及Hcy的联合检测在CHD诊断中展现出较高的灵敏度和特异度,为CHD的早期诊断提供了新的生物标志物组合,具有重要的临床应用价值。Objective To explore the combined application value of blood lipid,cystatin C(Cys C)and homocysteine(Hcy)in the diagnosis of coronary heart disease(CHD).Methods The clinical data of eighty-one patients with CHD who were treated in the Binzhou Medical College Affiliated Hospital from March 2022 to March 2024 were retrospectively selected as the inquiry group,and the clinical data of ninety-seven healthy subjects were collected as the control group.Blood lipid,Cys C and Hcy were detected.The differences in the levels of each index between the two groups were compared,and the diagnostic efficacy of the above indexes alone and in combination was evaluated.Results The levels of blood lipid,Cys C and Hcy in the inquiry group were higher,and the differences were statistically significant(all P<0.05).The diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined detection were 97.53%(79/81),96.91%(94/97),97.19%(173/178),96.34%(79/82)and 97.92%(94/96),respectively,which were higher than those of single detection,and the differences were statistically significant(χ^(2)=6.767,7.339,8.021,6.823,7.142,all P<0.05).Conclusion The combined detection of blood lipids,Cys C and Hcy showed high sensitivity and specificity in the diagnosis of CHD,which provided a new biomarker combination for the early diagnosis of CHD and had important clinical application value.
