气管插管全麻联合臂丛神经阻滞在肩关节镜手术中的应用分析

摘　　要：目的探讨气管插管全麻联合臂丛神经阻滞用于肩关节镜手术麻醉中的效果与价值。方法非随机选取2022年3月—2023年9月在盱眙县人民医院行肩关节镜手术治疗的患者79例为研究对象,按麻醉方法不同分为两组,对照组(39例)采取气管插管全麻方式麻醉;观察组(40例)采取气管插管全麻联合臂丛神经阻滞方式麻醉。对比两组心率、血压指标、硝酸甘油及麻醉药物使用剂量,统计不良反应发生率。结果麻醉后30 min后及麻醉后1 h,观察组心率(75.96±5.17)次/min、(79.08±5.13)次/min及平均动脉压(78.54±5.17)mmHg、(80.09±5.11)mmHg较对照组的(65.06±5.13)次/min、(68.92±5.14)次/min及(64.02±5.11)mmHg、(73.22±5.06)mmHg稳定,差异有统计学意义(t=9.405、7.184、12.552、6.003,P均<0.05)。观察组硝酸甘油及麻醉药物使用量均少于对照组,差异有统计学意义(P均<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论气管插管全麻联合臂丛神经阻滞方式麻醉用于临床肩关节镜手术治疗效果更显著,能够降低患者心率、血压值,减少麻醉药物使用量,同时不会增加不良反应。Objective To explore the effect and value of tracheal intubation combined with brachial plexus block in anesthesia for shoulder arthroscopic surgery.Methods Seventy-nine patients who underwent shoulder arthroscopic surgery at Xuyi County People's Hospital from March 2022 to September 2023 were non-randomly selected as the study subjects.They were divided into two groups according to the different anesthesia method.The control group(thirty-nine cases)received general anesthesia through tracheal intubation;The observation group(forty cases)was anesthetized with tracheal intubation combined with brachial plexus block.Compare heart rate,blood pressure indicators,dosage of nitroglycerin and anesthetic drugs,and calculate the adverse reactions rate in both groups of patients.Results 30 minutes after anesthesia and 1 hour after anesthesia,the heart rates of the observation group were(75.96±5.17)times/min,(79.08±5.13)times/min,and the mean arterial pressures were(78.54±5.17)mmHg,(80.09±5.11)mmHg,which were stable compared to the control group's(65.06±5.13)times/min,(68.92±5.14)times/min,and(64.02±5.11)mmHg,(73.22±5.06)mmHg.The differences were statistically significant(t=9.405,7.184,12.552,6.003,all P<0.05).The usage of nitroglycerin and anesthetic drugs the observed values of patients were smaller compared to the control patients,the differences were statistically significant(all P<0.05).There was no statistical significant difference in total probability of adverse reactions between two groups(P>0.05).Conclusion The combination of tracheal intubation general anesthesia and brachial plexus block anesthesia has a more significant effect on clinical treatment of shoulder arthroscopic surgery.It can stability heart rate and blood,reduce the use of anesthetic drugs,and do not increase adverse reactions.
