摘  要:目的评价在临床糖尿病诊断中采取血清C肽联合糖化血红蛋白检验的效果和准确度。方法非随机选取2023年7月—2024年8月贵阳市第一人民医院检验科接收的150例糖尿病确诊患者为观察组,另以同期在本院进行体检的150名健康者作为对照组;对比两组血清C肽、糖化血红蛋白水平,并分析单一及联合检测的诊断结果与诊断效能。结果观察组糖尿病患者血清C肽、糖化血红蛋白水平较对照组高,差异有统计学意义(P均<0.05)。150例糖尿病病例,血清C肽检出真阳性、真阴性例数分别是72例和138例;糖化血红蛋白检出真阳性、真阴性例数分别是80例、137例;血清C肽+糖化血红蛋白检查出真阳性患者98例、真阴性患者148例。血清C肽+糖化血红蛋白对糖尿病诊断的准确度为82.00%(246/300)、灵敏度为65.33%(98/150)及特异度为98.67%(148/150),均高于血清C肽的70.00%(210/300)、48.00%(72/150)、92.00%(138/150)与糖化血红蛋白的72.33%(217/300)、53.33%(80/150)、91.33%(137/150),3种检测方法的诊断效能比较,差异有统计学意义(χ^(2)=13.858、9.576、8.747,P均<0.05)。结论糖尿病患者的血清C肽、糖化血红蛋白增高,两项指标联合检验在糖尿病临床诊断中的应用可以弥补单一检测之不足,能够提高诊断准确度。Objective To evaluate the effect and accuracy of serum C peptide combined with glycosylated hemoglobin test in clinical diagnosis of diabetes.Methods From July 2023 to August 2024,a total of 150 diabetes patients diagnosed at the Laboratory Department of The First People's Hospital of Guiyang were selected non-randomly as the observation group.Additionally,150 healthy individuals who underwent routine physical examinations at the same hospital during the same period were chosen as the control group.The study compared serum C-peptide and glycosylated hemoglobin levels between the two groups,and analyzed the diagnostic outcomes and the diagnostic effectiveness of single versus combined testing methods.Results The levels of serum C-peptide and glycosylated hemoglobin in diabetic patients in the observation group were higher than those in the control group,and the differences were statistically significant(both P<0.05).In 150 cases of diabetes,the true positive and true negative cases of serum C peptide were 72 cases and 138 cases,respectively.The true positive and true negative cases of glycosylated hemoglobin were 80 cases and 137 cases respectively.Serum C-peptide+glycated hemoglobin was detected in 98 patients with true positive and 148 patients with true negative.The accuracy,sensitivity and specificity of serum C-peptide+glycosylated hemoglobin in the diagnosis of diabetes were 82.00%(246/300),65.33%(98/150)and 94.67%(148/150),respectively,which were higher than 70.00%(210/300),48.00%(72/150),92.00%(138/150)of serum C-peptide and 72.33%(217/300),53.33%(80/150),91.33%(137/150)of glycosylated hemoglobin,comparison of diagnostic efficiency of the three detection methods,the differences were statistically significant(χ^(2)=13.858,9.576,8.747,all P<0.05).Conclusion The serum C-peptide and glycosylated hemoglobin of diabetic patients were increased.The application of the combined test of the two indicators in the clinical diagnosis of diabetes can make up for the deficiency of single detection and improve the diagno
