PCT、CRP和WBC计数联合检测在小儿感染性肺炎诊断中的应用价值研究

摘　　要：目的研究降钙素原(procalcitonin,PCT)、C反应蛋白(C-reactive protein,CRP)、白细胞(white blood cell,WBC)计数联合检测在小儿感染性肺炎患儿临床诊断中的价值。方法非随机选取2023年1—10月段岳阳市中心医院收治的140例疑似小儿感染性肺炎的患儿作为研究对象,以病原菌检测作为金标准,对比PCT、CRP和WBC单一及联合检测的诊断结果及诊断效能。结果金标准结果表明小儿感染性肺炎阳性90例,阴性50例;PCT、CRP和WBC计数联合检测灵敏度98.89%(89/90)、特异度100.00%(50/50)、准确度99.29%(139/140)、阳性预测值100.00%(89/89)、阴性预测值98.04%(50/51)高于单一检测,差异有统计学意义(P均<0.001)。结论在为小儿感染性肺炎患儿诊断检查时,应用PCT、CRP和WBC计数联合检测,可有效诊断感染性肺炎患儿,诊断效能较高,相比于单一指标检测具有较高的诊断价值。Objective To study the diagnostic value of procalcitonin(PCT),C-reactive protein(CRP)and white blood cell(WBC)counting combined detection in the clinical diagnosis of pediatric infantile pneumonia.Methods From January to October 2023,140 children with suspected pediatric infective pneumonia admitted to Yueyang Central Hospital were non-randomly selected as the study objects,and the detection of pathogenic bacteria was used as the gold standard.The detection results and diagnostic efficiency of single and combined tests of PCT,CRP and WBC were compared.Results The gold standard results showed that 90 children were positive and 50 children were negative.The sensitivity of combined test of PCT,CRP and WBC counts 98.89%(89/90),specificity 100.00%(50/50),accuracy 99.29%(139/140),positive predictive value 100.00%(89/89),and negative predictive value 98.04%(50/51)were higher than those of the single test,the differences were statistically significant(all P<0.001).Conclusion In the diagnosis and examination of pediatric infective pneumonia in children,the combined detection of PCT,CRP and WBC count can effectively diagnose infective pneumonia in children,with high diagnostic efficiency,and has higher diagnostic value than single index examination.
