摘 要:目的分析异常凝血酶原联合血清α-L-岩藻糖苷酶,甲胎蛋白检测在肝细胞癌诊断中的应用。方法非随机选取盐城市第一人民医院于2022年2月—2024年3月收治的113例肝细胞癌患者作为研究对象,设为研究组,并选取同期的113名健康体检者作为对照组。比较两组研究对象异常凝血酶原、血清α-L-岩藻糖苷酶、甲胎蛋白水平的差异;比较异常凝血酶原、血清α-L-岩藻糖苷酶、甲胎蛋白3项指标单独和联合检测在肝细胞癌诊断中的检验效能。结果研究组各项检测结果均高于对照组,差异有统计学意义(P均<0.05)。检查结果显示,异常凝血酶原检测阳性120例,血清α-L-岩藻糖苷酶检测阳性116例,甲胎蛋白检测阳性119例,联合检测阳性115例。联合检测的准确度为97.35%(220/226),高于异常凝血酶原的75.66%(171/226)、血清α-L-岩藻糖苷酶的79.20%(179/226)、甲胎蛋白的78.76%(178/226),差异有统计学意义(χ^(2)=46.173,P<0.05)。结论通过异常凝血酶原、血清α-L-岩藻糖苷酶、甲胎蛋白联合检测,可充分发挥3种标志物各自诊断优势,提高诊断灵敏度和特异度,降低漏诊和误诊风险,这种联合检测方法可更全面地评估患者疾病状态。Objective To analyze abnormal prothrombin combined with serumα-L-fucoidase and alpha-fetoprotein detection in the diagnosis of hepatocellular carcinoma.Methods A total of 113 patients with hepatocellular carcinoma admitted to Yancheng First People's Hospital from February 2022 to March 2024 were non-randomly selected as the study group,and 113 healthy subjects in the same period were selected as the control group.The differences of abnormal prothrombin,serum α-L-fucosidase and alpha-fetoprotein levels between the two groups were compared.The test efficacy of single and combined detection of abnormal prothrombin,serum α-L-fucosidase and alpha-fetoprotein in the diagnosis of hepatocellular carcinoma was compared.Results The test results of the study group were higher than control group,and the differences were statistically significant(all P<0.05).Inspection results show,120 cases were positive for abnormal prothrombin,116 cases were positive for serum α-L-fucosidase,119 cases were positive for alpha-fetoprotein,and 115 cases were positive for combined detection.The accuracy of combined detection was 97.35%(220/226),which was higher than 75.66%(171/226)of abnormal prothrombin,79.20%(179/226)of serum α-L-fucosidase,and 78.76%(178/226)of alpha-fetoprotein,the difference was statistically significant(χ^(2)=46.173,P<0.05).Conclusion The combined detection of abnormal prothrombin,serum α-L-fucosidase and alpha-fetoprotein can give full play to the respective diagnostic advantages of the three markers,improve diagnostic sensitivity and specificity,and reduce the risk of missed diagnosis and misdiagnosis.This combined detection method can more comprehensively evaluate the patient's disease status.
