摘　　要：目的探讨髂静脉支架在症状性髂静脉狭窄或闭塞病变治疗中的有效性及安全性。方法收集2021年3月至2021年9月于济宁市第一人民医院进行髂静脉支架植入术的32例症状性髂静脉狭窄或闭塞病变患者的临床资料。统计所有患者的静脉功能评分,包括不同时间(术前、术后6个月、术后12个月)的临床表现病因学解剖学病理生理学(CEAP)分级、静脉临床严重程度评分(VCSS),并分析术后6、12、24个月的靶病变管腔一期通畅率。结果术后6、12个月CEAP分级均较术前有所下降,VCSS较术前有所改善,差异均有统计学意义(P﹤0.05)。术后6个月髂静脉彩色多普勒超声检查提示靶病变管腔一期通畅率为100%,术后12个月数字减影血管造影(DSA)下进行的下肢静脉顺行性造影显示靶病变管腔一期通畅率为93.7%,术后24个月髂静脉彩色多普勒超声检查显示靶病变管腔一期通畅率为92.6%。结论该临床试验静脉支架使用安全、有效并具有良好的临床通畅率。Objective To investigate the effectiveness and safety of the iliac vein stents in the treatment of symptomatic iliac vein stenosis or occlusion.Method The clinical data of 32 cases symptomatic iliac vein stenosis or occlusion patients who underwent iliac vein stent implantation surgery at the Jining No.1 People's Hospital from March 2021 to September 2021 was collected.The venous function scores of all patients were counted,including clinicaletiologyanatomy pathophysiology(CEAP)classification,venous clinical severity score(VCSS)at different time points(before surgery,6 months after surgery,and 12 months after surgery),and the primary patency rate of the target lesion lumen at at 6,12 and 24 months after surgery were analyzed.Result At 6 and 12 months after surgery,the CEAP classification decreased compared to preoperative CEAP classification,while the VCSS improved compared to preoperative VCSS,and the differences were statistically significant(P<0.05).Six months after surgery,color Doppler ultrasound examination of the iliac vein showed primary patency rate of 100%in the target lesion lumen,twelve months after surgery,digital subtraction angiography(DSA)performed anterograde angiography of the lower extremity veins showed primary patency rate of 93.7%in the target lesion lumen,twenty-four months after surgery,color Doppler ultrasound examination of the iliac vein showed primary patency rate of 92.6%in the target lesion lumen.Conclusion This clinical trial shows that the stent is safe,effective,and has good clinical patency rates.
