摘 要:目的探讨慢性阻塞性肺疾病(COPD)患者血清白细胞介素-6(IL-6)、血清淀粉样蛋白A(SAA)、呼出气一氧化氮(FeNO)水平表达的意义,并分析其对患者预后的影响。方法以100例COPD患者为研究对象,根据患者的病情程度分为稳定期组(50例)和急性加重期组(50例)。根据预后情况将稳定期组患者分为预后良好组(30例)和预后不良组(20例)。比较稳定期组、急性加重期组患者血清IL-6、SAA、FeNO水平。比较稳定期患者中预后良好组和预后不良组患者血清IL-6、SAA、FeNO水平。分析血清IL-6、SAA、FeNO水平与COPD患者预后的相关性。选用受试者工作特征(ROC)曲线,记录ROC曲线下面积(AUC)、约登指数(YI)及95%置信区间(95%CI),分析血清IL-6、SAA、FeNO水平对COPD患者预后的预测效能。结果急性加重期组血清IL-6、SAA、FeNO水平分别为(75.42±15.46)pg/ml、(56.38±10.76)mg/L、(59.38±6.54)ppb,显著高于稳定期组的(37.62±10.36)pg/ml、(22.16±6.71)mg/L、(39.23±7.69)ppb(P<0.05)。稳定期预后不良组患者血清IL-6、SAA、FeNO水平分别为(39.46±4.35)pg/ml、(24.31±4.02)mg/L、(41.36±5.46)ppb,显著高于预后良好组的(36.40±5.41)pg/ml、(20.72±3.28)mg/L、(37.81±4.23)ppb(P<0.05)。Pearson相关分析结果显示,血清IL-6、SAA、FeNO的表达水平与COPD患者预后情况均呈正相关(r=0.359、0.683、0.434,P<0.05)。ROC曲线分析结果显示,血清IL-6、SAA、FeNO水平对COPD患者预后情况具有良好的预测价值(AUC=0.702、0.903、0.740,P<0.05)。结论COPD患者血清中IL-6、SAA、FeNO水平可反映疾病的炎症状态及气道炎症类型,对病情评估及治疗有指导意义。血清IL-6、SAA、FeNO水平升高提示病情较重,影响预后,积极监测有助于改善患者预后及生活质量。Objective To explore the significance of the expression of serum interleukin-6(IL-6),serum amyloid A(SAA)and fractional exhaled nitric oxide(FeNO)in patients with chronic obstructive pulmonary disease(COPD),and analyze their influence on the prognosis of patients.Methods 100 COPD patients were taken as the research subjects,and they were divided into stable group(50 cases)and acute exacerbation group(50 cases)according to the degree of illness.Patients in stable group were divided into good prognosis group(30 cases)and poor prognosis group(20 cases)according to the prognosis of patients.The serum IL-6,SAA and FeNO levels were compared between stable group and acute exacerbation group,good prognosis group and poor prognosis group.The correlation of serum IL-6,SAA and FeNO levels with the prognosis of COPD patients was analyzed.The area under the receiver operating characteristic(ROC)curve(AUC),Youden's index(YI)and 95%confidence intervals(95%CI)of the subjects were recorded to analyze the predictive efficacy of serum IL-6,SAA,and FeNO levels on the prognosis of COPD patients.Results The levels of IL-6,SAA and FeNO in acute exacerbation group were(75.42±15.46)pg/ml,(56.38±10.76)mg/L and(59.38±6.54)ppb,which were significantly higher than(37.62±10.36)pg/ml,(22.16±6.71)mg/L and(39.23±7.69)ppb in stable group(P<0.05).The levels of IL-6,SAA and FeNO in poor prognosis group were(39.46±4.35)pg/ml,(24.31±4.02)mg/L and(41.36±5.46)ppb,which were significantly higher than(36.40±5.41)pg/ml,(20.72±3.28)mg/L and(37.81±4.23)ppb in good prognosis group (P<0.05). Pearson correlation analysis showed that the expression levels of serum IL-6, SAA and FeNO were positively correlated with the prognosis of COPD patients (r=0.359, 0.683, 0.434;P<0.05). ROC curve analysis showed that serum IL-6, SAA and FeNO levels had good predictive value for the prognosis of COPD patients (AUC=0.702, 0.903, 0.740;P<0.05).
