老年糖尿病合并高血压患者采用不同药物治疗的效果研究

摘　　要：目的探讨在老年糖尿病合并高血压患者的治疗中采取不同药物治疗的临床效果。方法选取90例老年糖尿病合并高血压患者,采用随机数字表法分为对照组和研究组,每组45例。对照组采取二甲双胍联合硝苯地平控释片治疗,研究组在对照组用药的基础上联合瑞格列奈、厄贝沙坦治疗。对比两组患者治疗效果、血压[收缩压(SBP)、舒张压(DBP)]、血糖[空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、糖化血红蛋白(HbA1c)]、不良反应发生情况。结果研究组治疗总有效率95.56%较对照组的77.78%高(P<0.05)。治疗12周,研究组SBP(114.89±5.94)mm Hg(1 mm Hg=0.133 kPa)、DBP(81.27±3.82)mm Hg较对照组的(128.46±5.86)、(92.64±4.71)mm Hg低(P<0.05)。治疗12周,研究组FBG(6.17±0.93)mmol/L、2 h PBG(8.27±1.13)mmol/L、HbA1c(6.76±0.58)%较对照组的(7.34±1.08)mmol/L、(10.02±1.21)mmol/L、(8.14±1.03)%低(P<0.05)。两组不良反应发生率对比未见明显差异(P>0.05)。结论老年糖尿病合并高血压患者的治疗中,在采取二甲双胍联合瑞格列奈降糖治疗的同时联合硝苯地平与厄贝沙坦降压治疗,有效控制了血糖与血压水平,提高了整体治疗效果,安全性较高,值得推荐。Objective To explore the clinical effect of different drugs in the treatment of elderly patients with diabetes mellitus complicated with hypertension.Methods 90 elderly patients with diabetes mellitus complicated with hypertension were selected and divided into control group and study group by random numerical table,with 45 cases in each group.The control group was treated with metformin combined with nifedipine controlled release tablets,and the study group was treated with repaglinide and irbesartan on the basis of the control group.The treatment effect,blood pressure[systolic blood pressure(SBP),diastolic blood pressure(DBP)],blood glucose[fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PBG),glycosylated hemoglobin(HbA1c)]and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of 95.56%in the study group was higher than 77.78%in the control group(P<0.05).After 12 weeks of treatment,the study group had SBP of(114.89±5.94)mm Hg(1 mm Hg=0.133 kPa)and DBP of(81.27±3.82)mm Hg,which were lower than(128.46±5.86)and(92.64±4.71)mm Hg in the control group(P<0.05).After 12 weeks of treatment,the study group had FBG of(6.17±0.93)mmol/L,2 h PBG of(8.27±1.13)mmol/L,and HbA1c of(6.76±0.58)%,which were lower than(7.34±1.08)mmol/L,(10.02±1.21)mmol/L,and(8.14±1.03)%in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion In the treatment of elderly patients with diabetes mellitus complicated with hypertension,metformin and repaglinide hypoglycemic treatment combined with nifedipine and irbesartan hypoglycemic treatment can effectively control blood glucose and blood pressure levels,improve the overall treatment effect.It has high safety,and is worthy of recommendation.
