摘　　要：目的 探讨保留旋前方肌掌侧入路锁定钢板螺钉内固定术对桡骨远端骨折患者腕关节功能的影响。方法 回顾性分析2021年8月—2024年6月张家港健华医院收治的62例桡骨远端骨折患者的临床资料,根据手术方式的不同将其分为对照组(n=30)和观察组(n=32)。对照组采用传统Henry入路掌侧锁定钢板螺钉内固定术,观察组采用保留旋前方肌掌侧入路锁定钢板螺钉内固定术。比较两组的围术期指标、腕关节活动度、腕关节功能、并发症发生情况。结果 观察组手术时间、住院时间均短于对照组,术中出血量少于对照组,组间差异有统计学意义(P<0.05)。术后3个月,观察组旋前、旋后、尺偏、桡偏、背伸、掌屈活动度均大于对照组,改良Mayo腕关节功能评分各维度评分均高于对照组,组间差异有统计学意义(P<0.05)。观察组并发症发生率为3.13%,低于对照组的23.33%,差异有统计学意义(P<0.05)。结论 桡骨远端骨折患者行保留旋前方肌掌侧入路锁定钢板螺钉内固定术较传统Henry入路较传统Henry入路锁定钢板螺钉内固定术的手术时间短、术中出血量少、住院时间短,利于促进腕关节活动度及功能恢复,且并发症的发生较少。Objective To investigate the effect of preserving the pronator palmar approach for locking plate and screw internal fixation on wrist joint range of function in patients with distal radius fractures.Methods Retrospective analysis of clinical data of 62 patients with distal radius fractures admitted to Zhangjiagang Jianhua Hospital from August 2021 to June 2024,according to the different surgical methods,the patients were divided into the control group(n=30)and the observation group(n=32).The control group underwent the volar approach locking plate and screw internal fixation surgery through the traditional Henry approach,while the observation group underwent the locking plate and screw internal fixation surgery through the palmar approach with preservation of the pronator quadratus muscle.Compare the perioperative indicators,wrist joint range of motion,wrist joint function,and complications between two groups.Results The operation time and length of hospital stay in the observation group were both shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,and the differences between the groups were statistically significant(P<0.05).Three months after the operation,the ranges of motion of pronation,supination,ulnar deviation,radial deviation,dorsiflexion,and palmar flexion in the observation group were all greater than those in the control group,the scores of each dimension of the modified Mayo wrist joint function score in the observation group were all higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of complications in the observation group was 3.13%,which was lower than 23.33%in the control group,and the difference was statistically significant(P<0.05).Conclusion Patients with distal radius fractures,the locking plate and screw internal fixation surgery via the volar approach with preservation of the pronator quadratus muscle has a shorter surgical time,less intraoperative bl
