宫腔镜辅助下冷刀切除术与电切术治疗对子宫内膜息肉患者子宫动脉血流指标的影响对比

摘　　要：目的 对比宫腔镜辅助下冷刀切除术与电切术治疗对子宫内膜息肉患者子宫动脉血流指标的影响。方法 选取济南市济阳区人民医院2022年10月—2023年10月收治的60例子宫内膜息肉患者为研究对象,按随机数字表法将其分为对照组和观察组,各30例。对照组行宫腔镜辅助下电切术,观察组行宫腔镜辅助下冷刀切除术。比较两组的围术期指标、应激反应指标、子宫动脉血流指标、子宫内膜厚度、并发症发生情况。结果 观察组手术时间、住院时间均短于对照组,术后阴道出血量少于对照组,组间差异有统计学意义(P<0.05)。术后24 h,观察组去甲肾上腺素、皮质醇、前列腺素E_(2)水平均低于对照组,组间差异有统计学意义(P<0.05)。术后1个月,观察组搏动指数、阻力指数均低于对照组,组间差异有统计学意义(P<0.05)。术后3个月,两组子宫内膜厚度均薄于术前,且观察组薄于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为6.67%,低于对照组的26.67%,差异有统计学意义(P<0.05)。结论 与宫腔镜辅助下电切术相比,宫腔镜辅助下冷刀切除术治疗子宫内膜息肉患者可优化围术期指标,减轻应激反应,改善子宫动脉血流指标和子宫内膜厚度,减少并发症的发生。Objective Comparison of the effects of hysteroscopic assisted cold knife resection and electric resection on uterine artery blood flow indicators in patients with endometrial polyps.Methods Selecting 60 patients with endometrial polyps admitted to Jiyang District People’s Hospital from October 2022 to October 2023 as the research subjects,according to the random number table method,they were divided into a control group and an observation group,with 30 cases in each group.The control group underwent hysteroscopic assisted electric resection,while the observation group underwent hysteroscopic assisted cold knife resection.Compare perioperative indicators,stress response indicators,uterine artery blood flow indicators,endometrial thickness,and occurrence of complications between two groups.Results The surgery time and hospitalization time of the observation group were shorter than those of the control group,and the postoperative vaginal bleeding was less than that of the control group,the differences between the groups were statistically significant(P<0.05).Twenty-four hours after surgery,the levels of norepinephrine,cortisol,and prostaglandin E_(2) in the observation group were lower than those in the control group,the differences between the groups were statistically significant(P<0.05).One month after surgery,the Pulsatility Index and Resistance Index of the observation group were lower than those of the control group,the differences between the groups were statistically significant(P<0.05).Three months after surgery,the endometrial thickness in both groups were thinner than those before surgery,and the observation group was thinner than that of the control group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 6.67%,which was lower than 26.67%in the control group,the difference was statistically significant(P<0.05).Conclusion Compared with hysteroscopic assisted electric resection,hysteroscopic assisted cold knife resection can optimize periope
