精蛋白人胰岛素联合门冬胰岛素治疗对妊娠期糖尿病患者血糖水平及不良妊娠结局的影响

摘　　要：目的探讨精蛋白人胰岛素联合门冬胰岛素治疗对妊娠期糖尿病(GDM)患者的影响。方法选取2022年1月—2024年1月浙江省人民医院毕节医院收治的92例GDM患者为研究对象,采用随机数字表法将其分为对照组和观察组,各46例。对照组采用门冬胰岛素治疗,观察组在对照组基础上采用精蛋白人胰岛素治疗。对比两组的血糖水平、血糖达标时间、不良妊娠结局发生情况。结果治疗3个月后,两组空腹血糖(FBG)、餐后2 h血糖(P2 hBG)、糖化血红蛋白水平均低于治疗前,且观察组均低于对照组,差异有统计学意义(P<0.05)。观察组FBG达标时间、P2 hBG达标时间、FBG和P2 hBG同时达标时间均短于对照组,组间差异有统计学意义(P<0.05)。两组不良妊娠结局发生率对比,差异无统计学意义(P>0.05)。结论应用精蛋白人胰岛素联合门冬胰岛素治疗GDM患者的疗效确切,可稳定降低血糖,缩短血糖达标时间,且安全性较好。Objective To explore the effect of protamine human insulin combined with insulin aspart on patients with gestational diabetes mellitus(GDM).Methods A total of 92 patients with GDM admitted to Zhejiang Provincial People’s Hospital Bijie Hospital from January 2022 to January 2024 were selected as the research subjects,using a random number table method,they were divided into a control group and an observation group,with 46 cases in each group.The control group was treated with insulin aspart,while the observation group was treated with protamine human insulin on the basis of the control group.Compare the blood glucose levels,time to blood glucose target,and occurrence of adverse pregnancy outcomes between two groups.Results After 3 months of treatment,the fasting blood glucose(FBG),postprandial 2-hour blood glucose(P2 hBG),and glycated hemoglobin levels in both groups were lower than those before treatment,and the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The FBG to reach the standard,the time for the P2 hBG to reach the standard,and the time for both FBG and P2 hBG to reach the standard in the observation group were shorter than those in the control group,the differences between the groups were statistically significant(P<0.05).Comparison of the incidence of adverse pregnancy outcomes between two groups,the difference was not statistically significant(P>0.05).Conclusion The protamine human insulin combined with insulin aspart has a definite therapeutic effect on patients with GDM,which can stably reduce blood sugar,shorten the time for blood sugar to reach the standard,and has good safety.
